About this file

This is a Quality Assurance Surveillance Plan (QASP) for ambulance transportation services provided to the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2 under solicitation 36C24227Q0007. The QASP establishes monitoring procedures and performance standards for Advanced Life Support (ALS) and Basic Life Support (BLS) ambulance transports serving eligible Veteran patients. The Contracting Officer's Representative (COR) will conduct quality assurance activities and notify contractors of any performance deficiencies, with corrective action timeframes determined on a case-by-case basis.

The plan specifies seven key performance indicators with 100% acceptable quality levels for most measures except initial pick-up and delivery/wait times, which require 95% compliance. Requirements include 24/7 availability regardless of distance, timely patient pick-up and delivery monitored through daily observation and transportation logs, vehicle and equipment compliance validated through periodic review of maintenance records and physical inspections, driver fitness testing for drug and alcohol use with criminal background checks, patient safety incident investigation and reporting within one hour by telephone, patient complaint documentation within 24 hours, and billing accuracy verification through random sampling of trip tickets against invoices. Non-compliance may result in negative past performance ratings, loss of option period exercises, cure notices, or contract termination for default.
