About this file
This is a Quality Assurance Surveillance Plan (QASP) for reusable medical devices courier services between the VA Health System of the Ozarks' Fayetteville, AR main campus and Springfield, MO CBOC. The QASP outlines the systematic method for evaluating contractor performance, including monitoring procedures, roles and responsibilities of key personnel (CO and COR), and specific performance standards.
The QASP details six key performance areas that will be monitored through direct observation and periodic inspection: trained drivers, insurance compliance, operational vehicles, delivery tickets, temperature/humidity controls, and cross-contamination measures. All performance standards require 100% compliance and will be evaluated through observation and random inspections by the COR. Unacceptable performance requires the contractor to submit a corrective action plan, and performance evaluations will affect future contract opportunities with the agency. The QASP is described as a "living document" that can be revised with contractor coordination.
