PERFORMANCE WORK STATEMENT (PWS)
PARTS/LABOR SERVICE CONTRACT
CLEAN LASER CL1000 LASER ABLATION SYSTEM
1.0 General
1.1 Scope: This Performance Work Statement (PWS) provides the details, specifications / requirements of the contract for parts sourcing, servicing, annual preventative maintenance and diagnosing of issues related to the Clean Laser CL1000 Laser Ablation System. This will be twice per year, scheduled annually for three years.
This effort should also include provisions for 2 emergency service visits if needed and telephone support.
1.2 Background: This contract is for the service, diagnosing, and yearly required preventative maintenance on the Clean Laser CL1000 Laser Ablation System.
1.3 Period of Performance (PoP): The Period of Performance shall be one Base Year of 12 months and 2 12-month option years. The Period of Performance reads as follows:

1.3.3 Option year two: TBD
1.4 General Information:
1.4.1 Quality Assurance (QA): The Government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and acceptable quality level(s) (defect rate(s)).
1.4.2 Recognized Holidays: The following are recognized US holidays. The contractor shall not be required to perform services nor deliveries to the depot on these days, but shall be required to work on Monday through Thursday and non-RDO Fridays. Building access for repairs and maintenance activities will be from 0600 to 1530. Changes to this schedule shall be coordinated with LEAD COR.:

When an observed holiday falls on a Tuesday or Thursday, the Depot regularly schedules a closure on the corresponding Monday or Friday.
1.4.3 Place and Performance of Services: The contractor shall operate within operating hours of Letterkenny’s standard work schedule of 0600-1530 - Monday through Friday. The exception to this is the alternating Friday Regular Day Off (RDO), Or on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. Performance for work under this contract shall be at Letterkenny Army Depot, 1 Overcash Ave, Chambersburg, PA 17201, particularly in support of the laser ablation processes and laser ablation equipment used to perform those processes. The contractor shall at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS when the Government facility/installation is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that stability and continuity of the work force is essential.
1.4.4 Security Requirements: Contractor and all associated sub-contractor’s employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce shall comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.
For contractors that do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
1.4.5 Physical Security: The contractor shall safeguard all Government property including facilities and equipment. At the close of each work period, Government facilities, equipment and materials shall be secured.
1.4.6 AT Level 1 Awareness Training: This standard language is for contractor employees with an area of performance within as Army controlled installation, facility or area. All contractor employees, including subcontractor employees, requiring access to Army installations, facilities, and controlled-access areas shall complete AT Level I awareness training within 30 calendar days after the contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 30 calendar days after completion of training by all employees and subcontractor personnel. 1.4.7 iWATCH Training: This standard language is for contractor employees with an area of performance within an Army controlled installation, facility or area. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity ATO). This local developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the COR NLT 30 calendar days after contract award. iWATCH training can be found at:https://www.letterkenny.army.mil/ctrresources/
1.4.8 OPSEC Training: For contracts that require OPSEC Training. Per AR 530-1 Operations Security, the contractor employees must complete Level I OPSEC Awareness training. New employees must be trained within 30 calendar days of their reporting for duty and annually thereafter. 1.5 Badges: Identification badges shall be issued for admittance of personnel before performing work or service on this contract. The identification badge is the property of the US Government and shall be returned upon termination or demand. The contractor/vendor shall ensure all badges issued to employees are returned to the Badge and Identification Section, Security Division, Letterkenny Army Depot promptly on termination of need or on expiration, which occurs first. The badge room is located in Bldg 2, phone # 717-267-5301. Hours of operation are 7:30 AM to 3:30 PM, Monday thru Thursday, exclusive of Federal Holidays.
1.5.1 Conservation of Utilities: The contractor shall instruct employees in utilities conservation practices. The contractor shall operate under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount.
1.5.2 Special Qualifications: The contractor shall ensure all employees possess all required licenses for operating any equipment used in the performance of this contract. NOTE: The Government does not provide training to contractors. Contractors must ensure that any personnel performing under a contract are fully trained, licensed, certified and qualified for the position in which they will be serving. The contractor shall submit with the solicitation a certificate or license certifying that the vendor is a certified and or licensed repair location for the manufactures listed in this PWS.
1.5.3 Post Award Conference/Periodic Progress Meetings: The contractor agrees to attend any post award conference convened by the Contracting Officer (KO) in accordance with Federal Acquisition Regulations Subpart 42.5. The KO, Contracting Officer’s Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contactor’s performance. At these meetings, the Contacting Officer will apprise the contractor of how the Government views the contractor’s performance and the contractor shall apprise the Government of problems, if any, being experienced. The contractor shall resolve outstanding issues raised by the Government. Contractor attendance at these meetings shall be at no additional cost to the Government.
1.5.4 Contract Manager (CM): The contactor shall provide a CM who shall ensure performance under this contract. The name of this person, and an alternate who shall act for the contractor when the CM is absent, shall be designated in writing to the KO. The CM or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.
1.5.5 Identification of Contractor Employees: All contact personnel attending meetings, answering Government telephones and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. The contractor shall also ensure that all documents or reports produced by contractor personnel are suitably marked as contractor products or that contractor participation is appropriately disclosed. If visiting, all contractor personnel will be required to contact the COR and will be required to meet all security requirements and regulations which are provided by the COR for site visit.
Requirements for a visit to the depot to support this contract are nonexistent.
1.5.6 Organizational Conflict of Interest Clause: Any conflict of interest on the part of the Contractor shall be immediately disclosed to the Company. LEAD shall, its option, terminate the agreement if it finds that a conflict of interest exists and poses a material conflict to and with the performance of the Contractor's obligations.
1.6 Health and Safety:
1.6.1 The Contractor shall comply with applicable subparts of 29 CFR 1910, OHSAS ISO 45001, LEAD’s Safety and Occupational Health Program, LEAD 385-1, and LIA ANSI Z136.1, Safe Use of Lasers.

1.7 Health and Safety Reporting
1.7.1 Immediately report any injuries to the LEAD COR.
1.7.2 Immediately report any laser incidents to the LEAD COR.
1.7.3 Immediately report any damage of LEAD-owned equipment or facilities to the LEAD COR and LEAD Police. Do not move equipment involved in a property damaging incident until LEAD Police release the scene.
1.7.4 Immediately report any unexpected disruptions to LEAD processes (power disruption, trigging an alarm, etc.) to the LEAD COR.
1.7.5 Immediately notify the COR if the Contractor determines a work procedure is unsafe and cease work until the LEAD Safety Office determines that the unsafe conditions have been corrected, the COR will notify the Contractor that personnel may resume work.
1.7.6 If the requirements of this contract change, the proponent of the contract must submit all changes to the LEAD Safety Office for review.
1.8 Health and Safety Training
1.8.1 Contractor employees shall be trained by the Contractor prior to beginning work for the tasks the Contractor employees are expected to perform and hazards that are known to be encountered.
1.8.2 Contractor employees shall be trained in the use of PPE before conducting work.
1.8.3 Contractor employees shall be trained on the hazardous material products they use or work around.
1.8.4 Contractor employees shall be trained on affected or authorized user lock out/tag out, as applicable.
1.8.5 Contractor employees shall be trained on laser hazards, PPE, and controls.
1.8.6 Contractor employees shall be trained on the use of powered industrial equipment and licensed before use, as applicable.
1.8.7 Contractor employees shall be trained on the health effects of lead, cadmium, chromium, and asbestos exposure.
1.8.8 Contractor employees shall be trained on the use of respiratory protection, as applicable.
1.9 Personal Protective Equipment (PPE)
1.9.1 The policy of LEAD’s safety office dictates entry into the location of work requires non-tinted eye protection, hearing protection, and toe protection at a minimum and it shall be worn at all times.
1.9.2 Laser PPE will be worn when operating laser ablation equipment. PPE includes adequate laser eyewear and gloves.
1.9.3 Hard hats are required to be worn when overhead hazards are present.
1.9.4 The Contractor will provide all required PPE.
1.9.5 Laser Protection
1.9.6 The Contractor will have a written, established, OSHA-compliant laser radiation program that Contractor employees adhere to.
1.9.7 The Contactor will comply with the requirements within LEAD 385-1.
1.9.8 Laser ablation equipment use will be dictated by LEAD policies and procedures.
1.9.9 Use of the laser ablation equipment will be conducted only in areas designated for laser use.
1.9.10 Required PPE will be worn at all times when operating laser ablation equipment.
1.10 Respiratory Protection
1.10.1 The Contractor shall have a written, established, OSHA-compliant respiratory protection program that Contractor employees adhere to.
1.10.2 Respiratory protection will be required when levels of dust exceed the Permissible Exposure Limit (PEL) or Short-Term Exposure Limit (STEL) for lead, cadmium, or chromium.
1.10.3 The Contactor will comply with the requirements within LEAD 385-1.
1.10.4 Respiratory protection equipment, medical evaluations, and fit testing will be provided by the Contractor.
1.11 Powered Industrial Truck (PIT)/Industrial Equipment Use
1.11.1 The Contractor will have a written, established, OSHA-compliant PIT program that Contractor employees adhere to, if PIT equipment is required to conduct work.
1.11.2 All PIT/Industrial Equipment, (including forklifts, lifting devices, etc.) needed to conduct work will be provided by the Contractor without exception.
1.11.3 Property damaging incidents while using PIT/industrial equipment will require immediate reporting to the LEAD COR and LEAD police.
1.11.4 A ground guide will be used to clear PIT/industrial equipment of obstacles within buildings when visibility is restricted.
1.11.5 The Contactor will comply with the requirements within LEAD 385-1.
1.12 Hazardous Material Management
1.12.1 The Contractor will have a written, established, OSHA-compliant hazard communication (HAZCOM) program that Contractor employees adhere to.
1.12.2 All chemical products brought onto LEAD property by the Contractor require a U.S.-based, English, current safety data sheet (SDS) to accompany it.
1.12.3A list of the hazardous materials products to be used and copies of SDSs will be provided to the LEAD COR, preferably in digital format.
1.12.4 Hazardous material containers will comply with OSHA HAZCOM requirements.
1.12.5 The Contactor will comply with the requirements within LEAD 385-1.
1.13 Fire Prevention:
1.13.1 The Contractor shall comply with appliable fire prevention regulations, codes, and requirements within 29 CFR 1910, NFPA 115, Standard for Laser Fire Protection, and NFPA 51B, Standard for Fire Prevention During Welding, Cutting, and Other Hot Work.
1.13.2 Hot Work
1.13.3 Any activity that produces heat, sparks, or flames will require an approved Hot Work Permit using the LEAD AMLD 4596 form.
1.13.4 Hot work permits will be approved by the LEAD fire department each day that hot work is conducted.
This daily requirement can only be reduced by the LEAD fire department and in writing.
1.13.5 Hot work attendants will be provided by the Contractor.
1.13.6 All hot work will be conducted in accordance with NFPA 51B.
1.13.7 The Contactor will comply with the requirements within LEAD 385-1 and LEAD 420-13, LEAD Fire Prevention Policy.
1.14 Environmental Protection and Pollution Prevention:
1.14.1 Pollution Prevention / Hazardous Waste Minimization: Contractor shall Minimize the use of hazardous materials and the generation of hazardous waste. Considerations and recommendations from supplier for greener products/ less hazardous waste generated and or consumed. Consult with the installation Environmental Office for suggestions and to obtain a copy of the installation's pollution prevention/hazardous waste minimization plan for reference material when preparing this part of the plan.
1.14.2 Compliance with Environmental Laws and Regulations: Contractor shall comply with all applicable federal, state, and local environmental laws, statutes, regulations, executive orders, permits, Army regulations (with supplements), as well as Major Subordinate Command (MSC) and installation regulations and policies.
Contractor shall immediately report any conflicts between applicable federal, state, local environmental laws, statutes, executive orders, and provisions of Army Regulation 200-1, and any specifications within this contract to the Contracting Officer Representative (COR) as well as the Director of Engineering & Public Works
1.14.3 Compliance with Green Procurement Requirements: Contractor shall follow Federal EPA Comprehensive Procurement guidelines (www.epa.gov/cpg) and Army Contracting Command Quick Guide (https://acc.aep.army.mil/accapps/ACCMAP/Documents/Quick-Guide-for-Sustainable-Procurement.docx) for acquisition of building materials and products and select materials that have a long life cycle; the least toxic materials; recyclable materials; materials that are resource-efficient; materials with the maximum recycled content; materials harvested on a sustained yield basis; and products causing the least pollution during their manufacture, use, and reuse.
1.14.4 Notification of Federal and State Regulators: Contractor shall immediately notify Director of Engineering & Public Works and COR of the arrival on site of any Federal, State, and/or DOD environmental regulator or enforcement agent and/or the receipt of any correspondence from a Federal or State environmental agency.
1.14.5 Inspections of Work Sites: Contractor shall submit to potential Federal, State, Army and installation work site environmental regulatory inspections and/or investigations into noncompliance, and fully cooperate with such inspections/investigations by providing the appropriate records and documentation. Environmental regulatory agencies are authorized by law to inspect any work site for environmental compliance with regulatory requirements. If an inspection is conducted, it will not stop or disrupt ongoing contract activities.
The inspection will only require the work site environmental officer, or supervisor/manager to answer questions and/or escort the inspector to specific work site areas with the potential to affect environmental quality. Typical environmental work site inspections are conducted in less than 30 minutes with an approximate frequency of one inspection every two months.
1.14.6 Reporting Noncompliance: Contractor shall immediately report any nonconformance and/or noncompliance with applicable Federal, State or Local environmental laws, Army and installation environmental regulations or policies to the COR and Director of Engineering & Public Works.
1.14.7 Conformance with Environmental Management System: Contractor shall take the necessary actions to identify, monitor, and control those contract operations and activities that pose risk of contamination, or can negatively affect the natural and/or human environment.
1.14.8 Competency Training for Contractor Personnel: Contractor shall not allow personnel to perform any activities and/or tasks on AMC installations without proper and adequate qualifications or job competency training. In the event of any identified noncompliance, the Contractor shall, if requested, provide proof of contract personnel training or qualification (individual name, training/qualification type, training/qualification certificate, and date of training/qualification) to perform those contract activities associated with the identified noncompliance.
1.14.9 Corrective Action for Noncompliance: Contractor shall when given a verbal and/or written notice of environmental noncompliance or nonconformance by the COR, take immediate corrective action. Failure or refusal to comply promptly may be grounds for the Contracting Officer to invoke the appropriate contractual remedies. This may cause all or part of the work to be stopped immediately until satisfactory corrective action has been taken.
1.15 Generation of Waste and Hazardous Waste: Contractor shall follow Letterkenny SOP for waste management covered in EMSP_21-17. This will be provided in the specification package.
2.0 Definitions and Acronyms:
2.1 CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime. Reference to Contractor includes sub- Contractors.
2.2 CONTRACTING OFFICER (KO). A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.
2.3 CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government designated by the KO to monitor contractor performance. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
2.4 DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.
2.5 DELIVERABLES. Anything that can be physically delivered and includes non-manufactured things such as meeting minutes or reports.
2.6 KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.
2.7 PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.
2.8 QUALITY ASSURANCE (QA). The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
2.9 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.
2.10 QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.
2.11 SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.
2.12 WORKDAY. The number of hours per day the Contractor provides services in accordance with the contract.
2.13 WORK WEEK. Monday through Friday, unless specified otherwise.

3.0 Government Furnished Property, Material, Equipment and Services (GFP/M/E/S): LEAD shall provide no equipment or property.
3.1 Government Responsibility:
3.2 LEAD personnel shall be responsible for providing a clean, dry area for the vendor to perform troubleshooting and maintenance activities to ensure the proper operation of the laser ablation system.
3.3 LEAD personnel shall be responsible for any inspections of the contractor repairs and preventative maintenance made to the laser ablation system to ensure the proper operation of the system.
3.4 LEAD personnel shall be responsible for any utilities that contractor repair technicians need to connect to equipment.
3.5 LEAD POC shall report non-compliance issues immediately to depot environmental and safety personnel upon notification from the contractor or area personnel.
3.6 Utilities: All utilities in the facility will be available for the contractors use in the performance of this contract.
4.0 Contractor Furnished Property, Materials, Equipment (CFP/M/E):
4.1 General: Except for those items specifically stated to be Government-Furnished in Paragraph 3., the contractor shall furnish everything required to perform these services as indicated in Paragraph 1.1.
4.2 Provide all utility and installation requirements 60 days prior to work beginning including voltage and amperage requirements.
5.0 Requirements:
5.1 Annual Service. The Contractor is required to arrive at Letterkenny Army Depot twice per year to perform annual service requirements on the Clean Laser CL1000 Laser Ablation System (serial number L22-1792), LFE-401 filter system (serial number A21470310011001), OSH 80 laser optic (serial number 22-01254), and effiSCAN laser optic (serial number 22-01253). Any filters, coolant, disinfectant, etc. as directed by yearly service requirements will be changed. Records of service, repair and technical read outs will be considered as a deliverable to COR via paper and email. A one-page report listing these items as replaced, inspected, or serviced with date and signature will be delivered to COR in paper and electronically. The following minimum requirements will occur at the yearly service:
5.2 Replace the particulate filter of the internal cooling device.
5.3 Replace the cooling fluid of the internal cooling device.
5.4 Disinfect the cooling circuit.
5.5 Replace the desiccant inside the resonator.
5.6 Perform an inspection and control of the overall laser system. Task may only be carried out by a cleanLASER service technician.
5.7 Check the laser system performance data. Task may only be carried out by a cleanLASER service technician.
5.8 Perform preventative maintenance measures according to inspection protocol. This includes the CL1000 ablation unit, the LFE-401 filter system, the OSH 80 laser optic, and the effiSCAN laser optic. Task may only be carried out by a cleanLASER service technician.
5.9 Provide a form that is similar to or the same as exhibit 3 to COR in paper and electronically.
5.10 Emergency Repairs. Troubleshooting of various emergency issues will be made available through this contract to ensure the Clean Laser CL1000 Laser Ablation System, LFE-401 filter system and attached laser optic devices (OSH 80 laser optic, and the effiSCAN laser optic) meet and maintain proper operation. The contractor shall return emergency repair service calls within 4 hours. If the machine is operational, the contractor shall provide phone technical support to solve the problem. If the problem cannot be fixed via phone technical support, the contractor shall dispatch a service technician on site within 24 hours. Repairs identified by emergency diagnostic services shall be coordinated with the COR, who shall authorize repair by the method most advantageous to the Government. The contractor shall coordinate arrival dates with the COR. This shall not be construed to mean that the contractor has exclusive rights to perform all emergency maintenance.
5.11 Non-Emergency Repairs. Troubleshooting of non-emergency issues will be made available through this contract to ensure the Clean Laser CL1000 Laser Ablation System, LFE-401 filter system and attached laser optic devices (OSH 80 laser optic, and the effiSCAN laser optic) meet and maintain proper operation. The contractor shall return non-emergency repair service calls within 24 hours. If the machine is operational, the contractor shall provide phone technical support to solve the problem. If the problem cannot be fixed via phone technical support, the contractor shall dispatch a service technician on site within 96 hours. Repairs identified by non-emergency diagnostic services shall be coordinated with the COR, who shall authorize repair by the method most advantageous to the Government. The contractor shall coordinate arrival dates with the COR.
This shall not be construed to mean that the contractor has exclusive rights to perform all non-emergency maintenance. The Government reserves the right to perform non-emergency repairs in-house as required.
5.12 Parts. The contractor shall supply all proprietary and specialized parts required to provide routine preventative maintenance, emergency repairs, and non-emergency repairs to the Clean Laser CL1000 Laser Ablation System, LFE-401 filter system and attached laser optic devices (OSH 80 laser optic, and the effiSCAN laser optic). The contractor shall provide a complete list of the parts needed for preventative maintenance and emergency repairs. The contractor shall provide parts list updates as needed. The COR shall approve all parts costing more than fifty dollars ($50) each that are parts required by Letterkenny Army Depot to perform preventative maintenance and emergency repairs on the Clean Laser CL1000 Laser Ablation System, LFE-401 filter system and the attached laser optic devices (OSH 80 laser optic, and the effiSCAN laser optic). The contractor shall provide original part invoice to COR. Funding for repair parts will be allocated on individual delivery orders. This shall not be construed to mean that the contractor has exclusive rights to all supply parts for emergency and non-emergency maintenance. The Government reserves the right to procure and warranty parts required for maintenance performed in-house by the most advantageous means.
5.13 Technical Support. The contractor shall provide technical support for emergency repairs, non-emergency repairs, equipment operational issues and replacement parts related to the Clean Laser CL1000 Laser Ablation System, LFE-401 filter system and attached laser optic devices (OSH 80 laser optic, and the effiSCAN laser optic). The contractor shall return emergency and non-emergency repair service calls within the time periods in sections 5.2 and 5.3. If the machine is operational, the contractor shall provide phone technical support to solve the problem. If the problem cannot be fixed via phone technical support, the contractor shall dispatch a service technician on site within the time periods in sections 5.2 and 5.3. The contractor shall identify part numbers need for repairs and routine maintenance activities.
5.14 Warranty. The contractor shall provide a warranty for the period of performance of this contract for repair parts, repair labor, troubleshooting and performance of emergency and non-emergency repairs. The warranty excludes consumables, and any parts damaged due to misuse, abuse, or accident. Consumables include laser optic protection windows, filters, resonator desiccant packs, and laser chiller coolant.
5.15 The contractor shall coordinate any/all work with LEAD. They cannot remove, unhook, de-energize, or turn off any systems without LEAD assistance and perform this with local rules and laws including OSHA lock out tag out program. Chemical changes of any kind must be coordinated and documented to LEAD POC’s.
5.16 Contractor shall obtain all license and certification required by Federal, State, and Local environmental laws and regulations necessary to adhere to the specifications of this contract. The Contractor shall submit all plans, notifications, reports, submittal documents, and fees required by Federal, State, and Local environmental laws and regulations to the appropriate Federal, State, and Local authority and/or agency as necessary to adhere to the specification of this contract. All required licenses and certifications required by Federal, State, and Local environmental laws and/or regulations shall be considered a contract deliverable upon award.
5.17 The contractor shall cleanup upon completion of the designated work. All disposals shall be in accordance with regular or hazardous waste procedures.
5.18 The contractor shall follow all equipment, work handling, and PPE requirements as specified in SDS and HMAG document provided per chemical.
5.19 The contractor shall allow annual/as needed inspections from the LEAD POC and provide documented feedback as to what work is performed each day.
