About this file

The document PUR 1355.docx is a Foreign Country of Concern Attestation form for a state or local government procurement process, specifically designed for entities submitting bids, proposals, or entering contracts that would provide access to personal identifying information. The form requires an officer or representative to certify that their organization is not owned, organized, or controlled by the government of a Foreign Country of Concern, as defined in Rule 60A-1.020, F.A.C.

The attestation form is a compliance document that requires the bidding entity to declare under penalties of perjury that they meet specific ownership and operational criteria related to foreign government involvement. The form includes fields for the entity's name, printed name of the authorized representative, their title, signature, and date of execution. This document appears to be part of a broader effort to ensure that contractors do not have significant ties to potentially problematic foreign governments when accessing sensitive personal information through government contracts.
