The form requires vendors to make representations regarding covered telecommunications equipment and services for procurement actions under the micro-purchase threshold. Vendors must disclose whether they provide or use telecommunications equipment from certain manufacturers, with specific requirements outlined in FAR provisions 52.204-24, 52.204-26, and contract clause 52.204-25.

The form includes four key questions for vendors to answer if they use prohibited covered telecommunications equipment: (1) percentage of communications platform using restricted technology, (2) description of equipment from mentioned manufacturers, (3) willingness to provide a plan to remove such equipment by 2022, and (4) willingness to provide a detailed list of current equipment. Vendors must conduct a reasonable inquiry, review the System for Award Management (SAM) for excluded parties, and have an authorized representative complete and sign the certification, indicating their compliance with telecommunications equipment restrictions.
