About this file
This is a Past Performance Questionnaire (PPQ) template for a VA Generator Physical Security Upgrades project at 1201 N Muldoon Rd, Anchorage, AK (Project #463-25-101). The PPQ is designed to be completed in two sections - Section 1 by the contractor providing past performance information (including contract details, project description, and relevance) and Section 2 by the client evaluator rating the contractor's performance across seven key areas: quality, schedule/timeliness, communication, management/personnel/labor, cost/financial management, subcontract management, and overall summary.
The evaluator rates performance using a scale from Outstanding to Unsatisfactory across these categories. The completed PPQ is to be returned to the contractor for inclusion in their SF330 submission rather than submitted directly to NCO 20. The PPQ template is related to solicitation 36C26025R0045 for A-E services being procured by the VHA Network Contracting Office 20 in Vancouver, WA, with PPQs not counting toward the 50-page limit for SF330 submissions.
