About this file

This is a Past Performance Questionnaire (PPQ) template for use with VA solicitation #36C26025R0045 for Generator Physical Security Upgrades in Anchorage, AK. The PPQ is designed to be completed in two sections: Section 1 by the contractor providing information about their company and the referenced contract, and Section 2 by the evaluator rating the contractor's past performance. The evaluation covers seven key areas: quality, schedule/timeliness, communication, management/personnel/labor, cost/financial management, subcontract management, and overall summary ratings.

The PPQ uses a four-tier rating scale (Outstanding, Above Average, Satisfactory, Unsatisfactory) and requires detailed comments for each evaluation area. The completed PPQs are to be incorporated into contractors' SF330 submissions and are not counted toward the 50-page limit. The questionnaire specifically notes that completed PPQs should be returned to the offeror for inclusion in their submission rather than sent directly to the VA contracting office.
