About this file

This Purchase Order Scope of Work (PMT-57) is issued by the Florida Department of Children and Families (DCF) for the State Mental Health Treatment Facilities (SMHTFs), soliciting bids for the purchase and delivery of milk to three specific facilities: Florida State Hospital in Chattahoochee, Northeast Florida State Hospital in MacClenny, and North Florida Evaluation and Treatment Center in Gainesville. The contract term is anticipated to run from September 1, 2025, through August 31, 2030, with a 5-year renewal option. Deliveries are required twice weekly (Tuesdays and Fridays) between 6:00 a.m. and 3:00 p.m., with specific requirements for milk temperature (34-38 degrees Fahrenheit), fortification with vitamins A and D, and a minimum shelf life of seven days prior to expiration.

The payment method is fixed rate, with no advance payments and invoicing conducted through MyFloridaMarketPlace (MFMP). Providers must be registered with MFMP and will be subject to a 0.7% transaction fee effective July 1, 2024. Financial consequences for non-compliance include a $100 penalty per instance of delivery failure and potential responsibility for price differences if replacement milk must be purchased. The contract emphasizes strict delivery specifications, including leak-proof containers, cleanliness, and temperature control, with the facilities reserving the right to refuse deliveries that do not meet specified requirements. No travel reimbursements will be provided under this contract, and the actual quantities ordered will be determined by the specific needs of each mental health treatment facility.

2. Governance Based on §287.057(19), Florida Statutes (F.S.), if applicable, Provider(s) under this Contract will be prohibited from participating in any other engagement that the Provider provides related content as this would create a conflict of interest. The term of the resulting Contract is subject to change based on the actual execution date of the resulting Contract.

3.2 In accordance with §287.058(1)(g), F.S., the Contract resulting from this procurement may be renewed for a period that may not exceed three years or the term of the resulting original Contract period, whichever is longer.
