About this file

The document is a survey template from the Illinois State Board of Education (ISBE) designed to collect feedback from first-time Professional Educator License (PEL) teaching program completers. The survey aims to gather detailed insights about educators' preparation program experiences, focusing on program effectiveness, instructional preparedness, and professional development. Participants are asked to provide demographic identifiers like their Illinois Educator Identification Number (IEIN) or last four digits of their social security number to enable secure data matching. The survey covers multiple sections, including program type, institutional details, alignment with professional standards, program strengths and weaknesses, student teaching experiences, and opportunities for professional growth.

The survey is anonymized and will be stored in a secure, password-protected database operated by ISBE. Aggregate, anonymous data will be shared with educator preparation programs to help ensure statewide program quality through the Illinois Educator Preparation Profile (IEPP). The survey takes approximately 7-10 minutes to complete and does not impact licensure eligibility. Respondents are asked to rate their agreement with statements about program preparedness, instructional design, assessment capabilities, collaboration skills, and professional expectations. The survey also solicits narrative feedback on program strengths and areas for improvement, with a specific focus on student teaching/residency experiences and opportunities to work in diverse educational settings.

PEL Teaching Completer Survey

The purpose of this survey is to provide the Illinois State Board of Education (ISBE) with information from program completers about their Educator Preparation Program experiences.

To reduce the length of this required survey, you will be asked to provide your date of birth and either your Illinois Educator Identification Number (IEIN) or last four digits of your social security number so ISBE can match your responses to your educator preparation program. This information will only be visible to ISBE. This information, along with your survey responses, will be stored in a secure, password-protected database operated by ISBE.

Aggregate, anonymous survey data will be shared with your educator preparation program and help ISBE ensure the quality of all programs in the state through the Illinois Educator Preparation Profile (IEPP.) Your anonymous narrative responses will be shared with your program so it can celebrate its successes and make improvements where needed.

Your responses to this survey will not impact your eligibility for licensure. Your responses will be used to help improve educator preparation in Illinois for future educators.

This survey should take 7-10 minutes to complete.

1. What is your Illinois Educator Identification Number (IEIN)? Your IEIN is located at the top of your Credentials page, beneath your name, in ELIS.
