About this file

This is a Pre-Construction Risk Assessment (PCRA) form for project 402-24-001 at the Togus VA Medical Center, submitted on 9/24/2024 and signed by infection prevention on 9/30/2024. The project involves investigating and potentially infilling underground vaults discovered in spring 2023 behind the Cabin in the Woods grounds.

The document details that the underground structure is currently only accessible via four chimney-like structures protruding from the ground and is completely covered by vegetation/hillside. The contractor must perform a full investigation of the structure's conditions and provide a report including dimensions, entry points, and materials, as well as assess the feasibility of infilling with gravel or alternative fill to prevent collapse. The assessment indicates this will be a pre-award contractor project requiring confined space entry, with work expected to take longer than a single shift but less than 6 months. The area is classified as vacant (outside work) with Mark Roberts from FMS listed as the point of contact. The project requires consultation with Infection Prevention and Control for ICRA completion.

402-24-001 Close and Infill Underground Vaults - PCRA

Request #244

Initiated by Abigail CampbellPre-Construction Risk Assessment (PCRA)

Togus Submitted 9/24/2024

Infection Prevention Review: Signed by vhatoghammv 9/30/2024

Behind Cabin-in-the-Woods

1.2: Activity Name, Number, and/or Brief Description *

402-24-001 Close and Infill Underground Vaults

In the spring of 2023 an underground structure was discovered on Togus campus located behind the Cabin in the Woods grounds, see Structure Location Map. The underground structure is entirely covered in vegetation/hillside and can currently only be accessed by (4) chimney-like structures protruding out of the ground. Size, make and history of the structure are unknown.

Contractor is to perform a full investigation of the underground structure conditions and to provide a final report with findings, including, but not limited to, structure dimensions, potential entry points, and structure materials. Report to also include the feasibility of infilling the underground structure with gravel or an alternative type of fill to prevent structure collapse.

Once the VA has reviewed and approved the proposed solution and materials from the initial report, the second portion of work includes all construction services to infill the structure.

3.2.1: If adjacent area is vacant (e.g., work outside, construction of new building, etc.): Coordination is typically not necessary other than potentially traffic flow and pedestrian access.

3.2.2: If adjacent area is non-continuously occupied (e.g., areas where outpatient care is provided, employee health, etc.):Develop a list of activities that will potentially impact or disrupt the operation of the area (e.g., work involving noise, vibration or exit obstruction) and meet with POC to coordinate execution of work in a way that mitigates the impact (e.g., conduct work after hours).

3.2.3: If adjacent area is occupied continuously (e.g., areas where inpatient care is provided, residential areas such as Community Living Centers, etc.):Develop a list of activities that will potentially impact or disrupt the operation of the area (e.g., work involving noise, vibration or exit obstruction) and meet with POC to coordinate execution of work in a way that mitigates the impact

(e.g., move affected party temporarily).

PREVENTION & CONTROL regarding the assessment of potential infection risks associated with the activity and the need for control measures. Project Lead must submit Infection Control Risk

Assessment (ICRA) for ALL projects. See VHA Directive 7715 and the VHA ICRA Template for more information.
