SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF
PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME
CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. In an effort to assist the
Government's Past Performance Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past
Performance Volume (or Proposal). (Note: Solicitation Section L – Instructions to Offerors, Volume 1, Past Performance should specify if past performance is to be submitted as a separate proposal volume).
