About this file
This is a Past Performance Report Form template used by NAVSUP Fleet Logistics Center Norfolk (FLCN) to evaluate contractor past performance for solicitation N0018926R3102, Admiral Gooding Center Events and Information Technology Support Services. The form is to be completed by government agencies or companies who have previously worked with a contractor under evaluation and returned to Tierra Watson, Code 233 at FLCN no later than the solicitation due date. The form requests information on the contractor being evaluated, including company name, contract number, contract amount, contract type, period of performance, and business address.
The evaluation criteria are organized into four assessment categories: Quality (recruiting ability, workforce retention, and meeting contract requirements including customer satisfaction, rework instances, and innovative applications); Timeliness (compliance with performance schedules and timely submission of reports and invoicing); Responsiveness (ability to address customer concerns, problem resolution effectiveness, and cost control); and Additional Observations (overall contractor strengths and weaknesses, satisfaction with contract award, awareness of similar contracted efforts, and recommendations for additional references). Respondents are instructed to complete the form on behalf of the company being evaluated and to provide additional pages if more comment space is needed. The form serves as source selection information and is not to be returned to the originating contractor.
