About this file

This document is a Past Performance Questionnaire for evaluating a contractor's previous performance for the Live Streaming and Jumbotron services for the 2025 Open House and Air Show (OHAS) at Joint Base McGuire-Dix-Lakehurst. The questionnaire is designed to collect detailed feedback from a previous client or project point of contact, with a comprehensive 14-point assessment covering various performance dimensions such as management quality, problem-solving, supervision, cooperation, and overall contract execution. The evaluation uses a 0-5 rating scale with definitions for each performance level, allowing the respondent to provide nuanced feedback about the contractor's technical capabilities, administrative skills, quality control procedures, and responsiveness to project requirements.

The questionnaire requests specific information about the contractor, including contract details, performance ratings across multiple categories, and additional contextual questions about potential contract renewal, discrepancy management, and whether the contract was part of an air show. The form is to be completed and returned to two Air Force contracting officers, Shawna Dancel and Lizbette Zalybniuk, via email. This evaluation will likely be used as part of the source selection process for the upcoming OHAS Jumbotron and Live Stream Broadcast solicitation (FA448425Q0014), which aims to provide multimedia communication capabilities for a 125,000-person crowd during the May 2025 event.
