About this file

This is a Past Performance Questionnaire for the Museum Exhibit Fabrications solicitation (HT0014-26-Q-E018) issued by the Defense Health Agency's Healthcare Contracting Division Northeast for the National Museum of Health and Medicine at Silver Spring, Maryland. The questionnaire must be completed and returned by August 13, 2026, at 10:00 AM EDT and serves as an evaluation tool for assessing offeror past performance as part of the source selection process.

The questionnaire requests detailed information about the evaluated contractor's prior performance across three primary categories: Quality of Services, Timeliness of Performance/Services, and Business Relations. Evaluators must provide adjectival ratings (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) for specific performance elements including product and service quality, personnel qualifications, contract management, quality control plans, project management, schedule adherence, budget tracking, file deliverables, subcontractor retention, communication effectiveness, and complaint resolution. The form requires respondents to provide substantiating comments for any rating other than Satisfactory and to disclose any cure notices, show cause notices, payment suspensions, or termination actions issued during contract performance. Respondents must identify the contractor being evaluated, provide contract details (including contract number, value, type, and performance period), identify the Contracting Officer's Representative, and provide an overall performance rating with supporting comments.
