About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on completed federal contracts. The form captures comprehensive information about the contractor, contract details, and performance metrics. It requires completion by the contracting agency or customer who directly worked with the contractor.

The questionnaire collects basic contract identification information including contractor name, address, contract number, procuring agency details, period of performance, and dollar amounts (award amount, annual amount, and total value with modifications). Respondents specify the contract type—Firm Fixed Price, Time & Materials/Labor Hour, Cost Reimbursement, or other arrangements—and provide the contract title and scope description. The evaluation section assesses seven performance factors: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance. Each factor requires mandatory written comments and a rating using adjectival standards (Unsatisfactory, Marginal, Satisfactory, Very Good, or Exceptional). The questionnaire concludes with a yes/no/maybe question regarding future contractor selection, signed and dated by the evaluator with their title provided. Accompanying rating guidelines define performance standards for each evaluation factor, providing detailed criteria for assigning each rating level based on compliance, accuracy, cost management, schedule adherence, responsiveness, and business conduct.
