About this file

This is a Past Performance Questionnaire template used by the National Oceanic and Atmospheric Administration (NOAA) to evaluate contractor performance on previous relevant contracts. The template is designed to collect structured feedback from government personnel who have direct experience administering or overseeing contractor work, with the ultimate goal of assessing past performance for a new solicitation (1305M326Q0301) concerning Santa Cruz Laboratory Cooling Tower and Chiller Maintenance and Servicing, which includes a base period plus four option years.

The questionnaire is divided into four sections: Sections I and II are completed by the offeror and major subcontractors, then provided to a government assessor for verification; Section III captures assessor information including name, title, role in contract administration, and period of involvement; and Section IV requires the assessor to provide an overall performance rating using five defined categories (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) based on contract compliance, quality and timeliness of delivery, cost management, communications, proactive management, and customer satisfaction. The completed questionnaire must be submitted to the contracting office via email, and offerors must submit copies of Sections I and II with their proposals by the solicitation closing date. This template is not specific to the related Point Adams Backup Power System solicitation but rather serves as a general tool for evaluating contractor past performance across NOAA procurements.
