About this file

The document is a Past Performance Questionnaire for evaluating a contractor's previous work performance, designed to help the Defense Logistics Agency (DLA) assess a potential contractor's capabilities for a Material Handling Equipment (MHE) Maintenance Services contract in Guam. The questionnaire requires detailed ratings across multiple performance categories including technical performance, management/key personnel, schedule compliance, small business utilization, and regulatory compliance, using an evaluation scale from Exceptional to Unsatisfactory.

The associated federal contract opportunity (Solicitation Number SP4510-25-Q-1003) is a 100% small business set-aside for MHE preventative and corrective maintenance services in Guam, with a 12-month base period and two 12-month option periods. The contract will be a Firm-Fixed Price, Time and Materials Indefinite Delivery/Indefinite Quantity (IDIQ) with a total contract length not exceeding three years. Quotes are due by 3:00 PM EST on June 20, 2025, with questions accepted until June 6, 2025. Past performance will be significantly more important than price in the evaluation, and offerors are required to submit past performance references with independent survey responses.
