About this file

This is a Past Performance Questionnaire template for evaluating contractor performance on federal contracts. The questionnaire is specifically designed for solicitation 1305M326Q0370, which concerns Janitorial Services for the National Oceanic and Atmospheric Administration (NOAA) Office of Law Enforcement in Ketchikan, Alaska, under the Department of Commerce.

The questionnaire is divided into four sections: Section I captures solicitation data including the project name and agency; Section II requires the offeror to provide historical contract information and allows the assessor to verify accuracy; Section III collects assessor information including their role in contract administration (such as Contracting Officer, Administrative Contracting Officer, COR, or Quality Assurance Specialist) and period of involvement; and Section IV requests an overall performance rating using five defined categories—Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable—with evaluation criteria based on contractual requirement fulfillment, quality and timeliness of service delivery, cost management, inter-party communications, proactive management, and customer satisfaction. Offerors must complete and submit questionnaires by the solicitation closing date to the contracting point of contact at jenna.taulman@noaa.gov.
