About this file

The document is a Past Performance Questionnaire template designed for contractors to provide detailed references for federal, state, local government, and private contracts. The form requires contractors to list at least three contracts completed within the last 5 years that are similar in size and scope to the contract being evaluated, with a preference for listing National Cemetery Administration (NCA) contracts first.

The template captures comprehensive contract details including contract number, contractor information, contract type, dollar value, award date, subcontracting percentage, work description, and contact person information. This questionnaire appears specifically related to the Department of Veterans Affairs National Cemetery Administration solicitation (QSE--36C78625Q50072) for grounds maintenance at Annapolis and Loudon Park National Cemeteries, which covers a base year from 9/2/2025 to 9/1/2026 with four additional option years.
