About this file

This is a Past Performance Evaluation Questionnaire template used by federal agencies to assess contractor performance on completed or ongoing contracts.

The questionnaire collects comprehensive contract identification information including contractor name, contract address, procuring agency, agency point of contact, contract type (Firm Fixed Price, T&M/LH, Cost Reimbursement, or other), contract dates, contract value, contract title, and detailed scope of work description. Evaluators assess contractor performance across six key factors: Quality of Work, Cost/Budget Control, Customer Satisfaction, Schedule/Timelines, Business Relations/Communications/Responsiveness, and Staffing/Personnel. Each factor uses a standardized rating scale—Quality of Work, Budget Control, Schedule/Timelines, Business Relations, and Staffing use a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory), while Customer Satisfaction uses a five-point Likert scale (1-Very Unsatisfied through 5-Very Satisfied). The questionnaire includes space for evaluator comments on each factor, evaluator identification information (name, title, date, signature), and sections for documenting contractor strengths and areas for improvement. The template concludes with a yes/no/with reservations question regarding whether the evaluator would recommend awarding another contract to the contractor. Comprehensive rating guidelines are provided, defining specific performance criteria and expectations for each rating level across all evaluation factors.
