About this file
This is an Ownership Disclosure Affidavit form required by the City of Cooper City, Florida for the Comprehensive Fee Studies contract opportunity. The affidavit mandates that proposing firms provide complete disclosure of ownership structures and interests in the contract. For corporations, firms must list the full legal names and business addresses of all officers, directors, and stockholders holding five percent or more of the company's stock. For trusts, the legal names and addresses of all trustees and beneficiaries must be provided. Additionally, the affidavit requires disclosure of any other individuals (excluding subcontractors, materialmen, suppliers, laborers, or lenders) who hold or will hold any legal, equitable, beneficial, or other interest in the contract or business transaction with the City. All addresses provided must be physical business addresses, as post office boxes are not acceptable.
The affidavit must be executed by an authorized representative of the proposing firm, with the affiant's signature, printed name, and date required for submission. This disclosure requirement is a standard municipal compliance document designed to ensure transparency and identify all parties with financial or beneficial interests in the proposed contract prior to award. Failure to accurately complete and submit this affidavit may result in proposal disqualification or contract termination.
