About this file
This is a vendor notification document from Solano Public Health regarding the Community Health Improvement Plan (CHIP) Implementation request for proposals. Solano Public Health is seeking qualified agencies to support implementation initiatives across four strategic health priorities: Behavioral Health Access to Care, Maternal and Infant Health, and Housing Stability. The scope of work includes funding systems-building initiatives, one-time training or technical assistance, and coordination of services. The implementation period is nine months, with a total funding allocation of up to $802,000 available through the RFP, distributed as up to $200,500 per strategic health issue.
The funding for this initiative is provided by Solano Public Health through state and local resources dedicated to community health improvement. Interested vendors were notified of the opportunity through this notification document dated May 30, 2023, allowing potential bidders time to review the comprehensive CHIP report and submission requirements. Additional information regarding the CHIP and its detailed goals can be accessed through Solano County's official documentation portal. Agencies interested in responding should be prepared to demonstrate expertise in systems-building, technical assistance delivery, and service coordination related to the identified health priorities.
