About this file

This Request for Quotation (RFQ) is issued by the West Virginia Department of Health and Human Resources, Bureau for Behavioral Health and Health Facilities, on behalf of Lakin Hospital to establish an open-end contract for the provision of medical oxygen and tanks. The solicitation (LSH1800000003) seeks bids to supply medical oxygen E tanks with a capacity of 24 cubic feet or equivalent on a continuing basis. The contract will be awarded to the vendor offering the lowest overall grand total cost that meets all mandatory specifications. Vendors must complete pricing pages detailing unit prices for exchange/refill rates per tank, daily rental rates per tank, and charges per delivery. Standard orders must be delivered within seven working days of receipt, while emergency orders require delivery within one working day. The document does not specify response deadlines, site visits, bidders' conferences, or award dates, nor does it identify the contract term or renewal options.

Pricing must include hazardous charges and delivery costs, with standard order delivery provided on an F.O.B. destination basis at no additional charge to the agency. The solicitation is partially or wholly funded by federal funds, requiring all bidders to acknowledge and adhere to provisions for federally funded procurements. The vendor must maintain sufficient inventory to fulfill contract obligations, designate a primary contract manager available during normal business hours, and provide quarterly reports and annual summaries detailing items purchased, quantities, and total dollar values. Vendor default provisions allow the agency to cancel the contract immediately or obtain items from a third party if delivery is delayed or specifications are not met. Returns of unacceptable items shall be at the vendor's expense with no restocking charge, while items returned due to agency error within 30 days may be subject to a restocking fee not to exceed 5% of the invoiced value.
