About this file

This is an Invitation for Bid offer form submitted to the State of Illinois, specifically for solicitation B-50387 related to UPS battery maintenance services at multiple locations. The contract opportunity, which has been canceled, was intended to establish a five-year agreement with the Illinois Department of Central Management Services (CMS) for uninterrupted power supply (UPS) inspection, repair, and maintenance across multiple facilities throughout the state. The form serves as the standard bidding document through which vendors formally submit their offers to perform work in compliance with the state's solicitation requirements and standard terms and conditions.

The offer form includes sections for bidders to declare any claims for Illinois-based business preferences, domestic product preferences, or exceptions to the standard contract terms and conditions, with instructions to submit redlined versions of any proposed modifications for state consideration. Bidders are required to complete all mandatory documentation as outlined in the solicitation's Section A.1 to avoid disqualification, and submission of the signed form constitutes a formal bid to the State of Illinois. The document references the state's standard terms and conditions available through the Illinois Procurement Office website, establishing the contractual framework that would govern any resulting agreement.
