About this file
This is a demolition signature page for an emergency residential demolition project located at 2733 Springwells (Building 102 Rear), also known as 2731 Springwells, in Michigan. The document establishes the bidding and submission requirements for contractors interested in this project. Bidders must submit their proposals under their full legal name, as this name will be used for purchase order issuance and payment. All contractors are required to visit the demolition site in person to inspect and review the property conditions before submitting a bid, and documentation of this site visit through a date-stamped photograph must be included with the bid response. The photograph must be the contractor's own documentation and cannot be the photograph provided in the Notice of Emergency Ordered Demolition.
The signature page requires submission of the contractor's Federal Employer ID number, total employee count, and the number of employees residing in Detroit. The authorized signatory must be a firm officer or an individual with documented authority to commit the organization to the bid, with the individual's name listed on the Certificate of Authority. Contact information including email, telephone, and cell phone number must be provided along with an alternate company contact. The document specifies that any contract assignment requires prior approval from the Chief Procurement Officer, the Chief Financial Officer, and potentially the City Council. This signature page was effective as of February 25, 2020.
