About this file

These documents are certification forms for a federal grant or contract opportunity, addressing three key compliance areas: telecommunications equipment restrictions, seat belt use policies, and distracted driving prevention. The first document, "Prohibition on Certain Telecommunications and Video Surveillance Services or Equipment Certification," prohibits bidders and subrecipients from using loan or grant funds to procure telecommunications or video surveillance equipment from specific foreign manufacturers like Huawei, ZTE, Hytera, Hikvision, and Dahua, based on national security concerns outlined in Public Law 115-232, section 889.

The subsequent two forms require bidders to certify compliance with specific executive orders and federal regulations. The "Seat Belt Use Certification" mandates that bidders adopt and promote on-the-job seat belt use policies for employees operating company or personal vehicles. The "Distracted Driving, Including Text Messaging While Driving Certification" requires bidders to implement workplace safety policies that ban text messaging while driving, conduct safety initiatives appropriate to their organization's size, and include similar provisions in third-party agreements. Each form provides space for a firm's details and requires an authorized person's signature, indicating the bidder's commitment to these federal guidelines.
