(e. g. freon, halon)?
26 Will there be modifications, destruction, or disturbance of the shoreline, streams, groundwater tables, surface water bodies, or wetlands? If so, explain.
Will there be any excavation and/or dewatering with the potential to impact the designated CT Coastal Zone (i.e., 1,000 feet inland from the MHW line)? If so, what type of activity will occur that can potentially impact the coastal zone area?
28 New proposed use and/or application of pesticides, rodenticides and herbicides? If so, indicate which type.
29 Will the project require pile driving?
OPERATIONAL RELATED QUESTIONS YES NO UNSURE
30 Will emission-generating equipment be installed (ex. paint booth, emergency generators)? If so, what type of equipment will be installed?

ADDITIONAL REQUIREMENTS or COMMENTS Comment 1 The proposed action is within the scope of CATEX #8: Routine repair and maintenance of buildings, facilities, vessels, aircraft, ranges, and equipment associated with existing operations and activities. None of the exclusions from reliance on a CATEX applies in this case; therefore, the proposed action does not require further NEPA analysis.
The applicability of this CATEX determination is based on the full implementation of all environmental requirements identified below.
Requir 1 Work occurs in the coastal zone; however, a Coastal Consistency Determination is not required provided that no deleterious material enters storm drains, manholes, or the Thames River.
Requir 2 Work occurs in or near an IR zone/site. However, no ground-disturbing activities are included in the scope of work. If ground-disturbing activities are planned, a dig permit shall be obtained prior to the initiation of such activity and soil management procedures detailed in the dig permit and the SUBASE Soil Management SOP shall be followed.
Requir 3 A list of HAZMAT scheduled for use or installation on the project and the approximate quantities/volumes of these materials shall be submitted to the PW NLON EV Division EPCRA Manager (x5140) prior to initiating project activities.
Requir 4 No wastewater shall be discharged to the storm drains or the ground surface. If wastewater discharges to the sanitary sewer are planned, the contractor shall obtain any required permits prior to discharging activity and shall coordinate in advance with the PW NLON EV Division Wastewater Program Manager (x2525).
Requir 5 If work activities require the use of pesticides or herbicides, the contractor shall coordinate in advance with the PW NLON Pesticide Program Manager (x5140).
Requir 6 Disposal of any regulated waste shall be coordinated in advance with the PW NLON Hazardous Waste Program Office (x4298).
