About this file

This document is a certification template issued by the United States Department of State for vendor compliance with the John S. McCain National Defense Authorization Act (NDAA) for Fiscal Year 2019.

The certification requires vendors to represent their compliance with federal requirements regarding covered telecommunications equipment and services. Vendors must complete two representations: first, whether they will provide covered telecommunications equipment or services to the Government in contract performance; and second, whether they currently use covered telecommunications equipment or services in their business operations. The form requires vendors to conduct reasonable inquiries before making these representations. If a vendor responds affirmatively to either representation, they must provide additional disclosure information as specified in FAR provisions 52.204-24 and 52.204-26 and contract clause 52.204-25. The completed certification must be signed by a merchant's authorized representative with binding authority and returned to the Contract Specialist. This is a template form with no specific products, services, response dates, or pricing terms specified.
