About this file
This is an Invitation for Bid (IFB) submission form for the State of Illinois, specifically for the NCL Irradiator Disposal project (Bid B-53599). The document serves as the official bid response template that vendors must complete and submit to offer their services or products to the state. The form requires bidders to declare their company information, confirm compliance with all solicitation requirements, and acknowledge that failure to meet stated requirements will result in disqualification. The document references a separate solicitation document that contains detailed specifications, timelines, and project requirements that bidders must review and address in their submissions.
The IFB form includes provisions for bidders to claim applicable preferences under the Illinois Procurement Code, including Illinois Business Preference and Domestic Product Preference, with supporting documentation required for validation. Bidders may also take exceptions to Standard Terms and Conditions by submitting a redlined version with their offer, subject to state acceptance or rejection. The Standard Terms and Conditions are available on the Illinois Chief Procurement Office website. The form contains multiple checkboxes and signature fields for authorized representatives to execute the bid, formally committing the bidder to perform in full compliance with the solicitation requirements if accepted by the State of Illinois.
