QUOTATION SUBMISSION FORM

Contractors that do not provide the required information below in its quote will not be considered for receipt of the contract. Contractors must complete and respond to every item in the table below to show its understanding of the requirement and ability to construct ladders in accordance with the specification. Contractors shall sign and return this attachment and any other required documents (i.e. certifications, pricing per Attachment 1, Schedule of Prices, etc.) with the quotation. The government will determine technical acceptability based on completeness and responsiveness of the contractors response to the requirements listed below. CAD drawing of ladder:

LIMITED TO 1 PAGE ONLY TO DEPICT (Ladder 34 inch (36 inch nominal width aisles) ONLY.

Single sheet single view CAD, three dimensional view CAD drawing: showing correct placement of all members and components. Correct placement of all members and components will be evaluated to show vendor understanding of design. The government will pay no costs for the drawing.

Include separate attachment. (Required)

Provide AWS certification (AWS D1.2/D1.2.3.5.2.1) for welders. If the welder is not AWS certified, provide documentation to show each welder’s experience, demonstrating a minimum of five (5) years experience in welding aluminum ladders or a minimum of ten (10) years of experience in welding aluminum shapes similar to ladder components.

*Contractor shall provide a copy of a certification or a list of experience for each welder performing work on this requirement)

Describe how quality control will be performed:

First Article – Address how you will perform weight testing in accordance with ANSI-ASC A14.7 - 2011 at your manufacturing facility.

All shipments – before shipment, inspect and verify weld quality, specification tolerances and proper operation of caster springs and casters, and that each ladder is free of burrs.

Include separate QA Plan attachment. (Required)
