PROPOSAL SUBMISSION & EVALUATION

1. INSTRUCTIONS FOR PREPARING QUOTATIONS

To be considered for award, offerors shall submit a complete quotation package consisting of the three (3) parts listed below. Failure to provide a complete package may result in your quote being deemed non-responsive.

Part I: Technical Proposal The technical proposal shall not exceed 5 pages and must include:

1. Proof of Licensing & Insurance: A copy of the company's valid California Structural Pest Control Business License. Offerors shall also provide a copy of their current liability and workers' compensation insurance certificates.

2. Technical Narrative: A brief narrative demonstrating an understanding of the PWS requirements, including your approach to managing routine and emergency service calls and your process for ensuring personnel meet all licensing and base access requirements.

Part II: Past Performance Provide a list of up to three (3) recent and relevant contracts performed within the last three (3) years that demonstrate the offeror's ability to perform work of similar scope and complexity to that described in the PWS.

1. Definitions:

"Recent" is defined as work that is currently active or was completed within the last three

(3) years from the issuance date of this RFQ.

"Relevant" is defined as work similar in scope, magnitude, and complexity to the requirements of this PWS. Experience on other Department of Defense (DoD) installations, other federal agency facilities, or large-scale commercial properties is considered relevant.

2. Submission Requirements: For each contract reference submitted, provide:

Customer/Client Name Contract Title & Number Period of Performance & Total Contract Value A customer point of contact (POC), including name, email, and phone number.

A brief narrative explaining the relevance of the work performed to this PWS.

The Government reserves the right to contact the provided references and may also use information from other sources (e.g., CPARS) to evaluate past performance.

Part III: Price Submit a completed and signed copy of the attached Price Schedule.

2. EVALUATION FACTORS FOR AWARD

(a) Basis for Award The Government intends to award a BPA to the responsible offeror whose quotation, conforming to the solicitation, represents the best value to the Government, price and other factors considered. This is a best value trade-off procurement. The Government reserves the right to award to other than the lowest priced quotation or the highest rated past performance.

(b) Evaluation Factors Quotations will be evaluated based on the following factors, listed in descending order of importance:

Factor 1: Technical Capability (Acceptable / Unacceptable) The Government will evaluate the technical proposal to ensure the offeror meets the minimum PWS requirements. To be rated "Acceptable," the proposal must include all requested documents (licenses, insurance) and the narrative must demonstrate a clear understanding of the work. An "Unacceptable" rating will make an offer ineligible for award.

Rating Description Acceptable Quotation clearly meets all minimum requirements of the solicitation Unacceptable Quotation does not clearly meet one or more minimum requirements of the solicitation

Factor 2: Past Performance The Government will assess its level of confidence in the offeror’s ability to successfully perform the required services. This assessment will be based on the relevance and quality of the offeror's recent past performance.

Performance Confidence Assessment Rating: Based on an integrated overall assessment of recency, relevancy, and performance quality, the Government will assign an integrated Performance Confidence Assessment Rating for each Offeror based on the table below. The Performance Confidence Assessment Rating represents the overall evaluation of contractor past performance.

Adjectival Rating Description Substantial Confidence Based on the offeror's recent/relevant performance record, the

Government has a high expectation that the offeror will successfully perform the required effort.

Satisfactory Confidence Based on the offeror's recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.

Neutral Confidence No recent/relevant performance record is available, or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or

Limited Confidence

Based on the offeror's recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort.

No Confidence

Based on the offeror's recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort.

In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance. Therefore, the Offeror shall be determined to have unknown past performance and will be assigned a performance confidence rating of "Neutral". A "Substantial" or "Satisfactory" confidence rating may be considered more advantageous to the Government than a "Neutral Confidence" rating.

Factor 3: Price The offeror's proposed prices in the Price Schedule will be evaluated for fairness and reasonableness. A Total Evaluated Price will be calculated for comparison purposes.
