PERFORMANCE WORK STATEMENT

PROJECT TITLE: Grease Trap Collection and Maintenance LOCATION: Roy M. Wheat Dinning Facility and Fire Stations 1 & 2 at Naval Air Station Meridian, Mississippi

1. GENERAL DESCRIPTION

Contract Line Item Numbers (CLINs) 1, 3, 5, 7, and 9:

The NASMER Roy M. Wheat Dinning Facility has a 1500‐gallon underground grease collection tank that requires routine maintenance. The contractor will be required to clean and treat the grease collection unit, as well as dispose of all contents extracted from the tank. Any debris removed from the tank must be transited off station to a proper disposal site. Maintenance services must be performed exactly one time every 30 calendar days (monthly). Each contract performance period is one year in duration which means a total of 12 service events are expected to occur during each contract year. The firm‐fixed price contract consists of one base year and four option years. The length of the contract shall not exceed 60 months. If the Government determines that the contractor performed satisfactorily during the active contract year and the customer conveys a continued need for services, then the Government will exercise the next option year. The Government will notify the contractor of its intent to exercise the next option year at least 30 days prior to contract expiration. The Contracting Officer will issue the next option year through a contract modification.

Contract Line Item Numbers (CLIN’s) 2, 4, 6, 8, and 10:

NASMER has two fire station facilities on the installation, Fire Station 1, and Fire Station 2. The facilities generate a relative amount of grease during daily meal preparation but neither facility has a mechanism in place to properly store and dispose of accumulated grease. The facilities are not co‐located, so each facility will require its own designated 50‐gallon grease collection unit. The units must be placed external to the facility but within walking distance to allow for the manual transfer of grease from the fire station kitchen area to the grease collection unit.

The contractor is required to supply two grease collection units that are rated for external use and possess a maximum capacity of 50 gallons. Containment devices capable of storing more than 50‐gallons are strictly prohibited. The contractor is responsible for the delivery and placement of units at locations specified by the Contracting Officer or the Contracting Officers Representative. Upon contract expiration the contractor will be required to retrieve and remove both units from the installation. In addition to supplying storage devices the contract shall routinely service both units at a frequency of one time every 60 calendar days for a total of six times per year. The contractor will be required to clean and treat the two portable grease collection units, as well as dispose of all contents extracted from the tank. Any debris removed from the tank must be transited off station to a proper disposal site.

2. DEFINITIONS

The terms used in these specifications shall have the meanings described below:

a. Grease Collection System: Includes the grease trap/grease interceptor, inlets, outlets, and airports on both indoor and in ground units.

b. Cleaning: Shall comprise of the complete removal and disposal of all fats, oils, grease, and all sunken food solids from the installed equipment to be serviced. All effluent or wastewater and all contents shall be removed. All areas outside of tanks contaminated by the hoses or grease during removal shall be cleaned with contractor furnished cleaning materials.

c. Contracting Officer: The Contracting Officer is the only Government representative authorized to initiate changes to the contract. If the Contracting Officer determines that a change to the contract is warranted, then the Contracting Officer will issue an official contract modification to the contractor for acknowledgement and signature. No other Government entities are authorized to alter the terms of the contract in any manner.

d. Performance Assessment Representative (PAR): The Contracting Officer designates a PAR, otherwise known as the Contracting Officers Representative, to oversee contract performance and implement quality control measures throughout the life of the contract. 3. SCOPE OF WORK

The contractor shall furnish all necessary labor, transportation, equipment, materials, supplies, and supervision to perform all cleaning operations related to removing fats, oils, grease, wastewater, and solids from the grease collection tanks. Contents of the grease collection tanks are to be pumped out of the tank and directly into a tanker/vacuum truck. The underground collection system is accessible by truck. The collection units to be supplied by the contractor will also be placed in an area that is accessible by truck. It is important to note that additional water may be needed to facilitate proper cleaning. The contractor is responsible for determining the proper cleaning method. However, the contractor and its representatives are strictly prohibited from entering the grease collection tank. In the event of a spill the contractor must immediately clean the affected area and report the incident to the NASMER Fire Department.

1. Any damage to the grease collection system by the contractor shall be immediately reported to the Contracting Officer and PAR. The contractor is financially liable for all damages and must restore the impacted area or object to its pre‐existing condition at no cost to the Government.

2. The contractor must certify that all chemicals utilized in performing the work described above will be non‐toxic, non‐corrosive, odorless, and non‐flammable.

3. Maintenance services are not to interfere with normal operations of the dinning facility or fire stations. If the contractor anticipates that the performance of services may interfere with normal dinning or fire station facility operations, then implementation of an after‐hours procedure is necessary. If the contractor determines that the performance of services may disrupt normal dinning or fire station facility operations the contractor must contact the PAR immediately to formulate an after‐hours maintenance procedure. An after‐hours maintenance procedure must be approved by the PAR in advance of the scheduled service date.

4. The contractor shall, without additional expense to the Government, obtain all license and permits required for the performance of the work.

5. The contractor shall provide a copy of the waste manifest to the NAVFAC SE NAS Meridian Non‐ Hazardous Solid Waste Manager Environmental Department for acknowledgement and signature.

If the contractor uncovers unforeseen deteriorated material of a major nature when performing maintenance services, do not disturb the material and immediately notify the Contracting Officer and PAR. Any measures taken to repair or remove or otherwise alter unforeseen conditions without the Contracting Officers direction are at the contractors expense.

5. WORK SCHEDULE

The contractor must provide a tentative performance work schedule no later than 10 days after contract award or 10 days after the start date of each contract year. The contractor must submit the schedule in writing to the to the Contracting Officer and PAR. Any request to work outside of normal working hours must be submitted in writing by the contractor to the Contracting Officer or a representative of the Contracting Officer. The contractor must submit a written request to work outside of normal working hours no less than 10 calendar days prior to the date that the contractor anticipates the work to take place. A minimum 10‐day notice affords sufficient time to the Government for arranging for the inspection of work and to accommodate any base access needs that may arise. Approval of such a request will be granted if working outside regular working hours is determined to be in the best interest of the Government.

6. FAR 52.236‐14: AVAILABILITY AND USE OF UTILITY SERVICES

(a) The Government shall make all reasonably required amounts of utilities available to the contractor from existing outlets and supplies, as specified in the contract. Unless otherwise provided in the contract, the amount of each utility service consumed shall be charged to or paid for by the contractor at prevailing rates charged to the Government or, where the utility is produced by the Government, at reasonable rates determined by the Contracting Officer. The Contractor shall carefully conserve any utilities furnished without charge.

(b) The contractor, at its expense and in a workman like manner satisfactory to the Contracting Officer, shall install and maintain all necessary temporary connections and distribution lines, and all meters required to measure the amount of each utility used for the purpose of determining charges. Before final acceptance of the work by the Government, the Contractor shall remove all the temporary connections, distribution lines, meters, and associated paraphernalia.

7. QUALITY CONTROL & SUPERINTENDENT DESIGNATION

The contractor is responsible for the quality of the work performed. Quality control reports are required each day that work is performed. The reporting of work shall be identified by terminology consistent with the service schedule. Contractor quality control reports are to be prepared, signed, and dated by the contractor’s appointed superintendent.

The prime contractor or a designated representative of the prime contract must always remain onsite during the performance of services. The contractor shall designate a superintendent in writing and submit the designation letter to the Contracting Officer 10 days prior to performing onsite work. The designation letter must detail the superintendent’s scope of responsibility, experience, qualifications, and authority to act on behalf of the contractor. If the prime contractor desires to change its superintendent designation, then the contractor must submit a new superintendent designation letter to the Contracting Officer and the PAR.

8. ACCIDENT PREVENTION

FAR 52.236‐13 Alt I is incorporated into this acquisition due to the nature of work to be performed. The contractor shall comply with the directives expressed in FAR 52.236‐13 to ensure the safety of the public, contractor personnel, Government property and Government personnel. The Contractor will maintain an accurate record of and will immediately report to the Contracting Officer and their appointed representative all mishaps resulting in death, traumatic injury, and occupational disease, as well as damage to property, materials, supplies, and equipment incident to work performed under this contract. The contractor will document in writing the details surrounding the workplace mishap on the “Contractor Incident Report System” form and submit the form electronically to the Contracting Officer and the PAR. The “Contractor Incident Report System” form is included as an enclosure. The accident‐reporting requirement does not relieve the prime contractor or its subcontracting affiliates of its responsibility to communicate verbally with the onsite Government representative upon securing the area where the mishap occurred.

The Contracting Officer will notify the contractor of any reports of accident prevention noncompliance and communicate any action taken by the Government to correct issues of noncompliance. Upon receipt of such notice the contractor shall immediately take corrective action. Such notice, when delivered to the contractor or their appointed onsite representative, shall be deemed sufficient for reporting purposes. If the contractor fails or refuses to take immediate corrective action, the Contracting Officer may issue an order stopping work until the contractor has taken satisfactory corrective action. The contractor shall make no part of the time lost due to any such stop orders the subject of claim for extension of time or for excess costs or damages. The prime contractor is responsible for ensuring that all subcontractors comply with the provisions outlined in this section.

To fulfill the requirements outlined in FAR 52.236‐13, the Contractor shall:

1. Submit in writing an Accident Prevention Plan (APP) & Activity Hazard Analysis (AHA) for review and approval by the Contracting Officer’s appointed representative, otherwise referred to as the PAR; (See EM‐ 385‐1‐1 for a sample APP & AHA)

2. Schedule meetings with the PAR to discuss and develop mutual understandings relative to the overall administration of the safety program.

9. WASTE MANAGEMENT

It is important to note that the accumulated grease to be disposed of in connection with this contract is a byproduct of the food preparation process and categorized as non‐hazardous solid waste.

Section 9 is standard template language used in contracts that involve the handling of solid waste. Handling of hazardous solid waste is not a functional requirement of the grease collection and maintenance services sought in solicitation N6945024R0029. Section 9a. should be considered informational in nature and any costs associated with handling or disposing of hazardous solid waste should be excluded from an offerors proposed price. Only the handling of non‐hazardous solid waste is a functional requirement of the grease collection and maintenance services sought in solicitation N6945024R0029. Costs associated with the handling or disposing of non‐hazardous solid waste should be included in an offerors proposed price.

Comply with all Federal, State, and Local regulations to include but not limited to the following:

a. Hazardous Solid Waste. All waste generated resulting from this service contract that meets the definition of hazardous waste, must be handled, accumulated, stored, transported, and disposed of in accordance with RCRA regulations for Large Quantity Generators (LOG). In accordance with the NAS Meridian Hazardous Waste Management Plan all hazardous waste shall be accumulated, in contractor‐supplied containers at approved accumulation sites. When containers are full or when work is complete the contractor will transfer accumulated waste, fill records, and Safety Data Sheets (SDS) to NAS Meridian’s less than 90‐day storage area for disposal. If the contractor encounters hazardous solid waste on the job site, the contractor is responsible for contacting and coordinating with the NAS Meridian Environmental Protection Specialist.

b. Non‐Hazardous Solid Waste. All non‐hazardous waste must be disposed of in regulated landfills in accordance with RCRA regulations. The contractor shall obtain the NAS Meridian Environmental Protection Specialist signature on manifest prior to departing the installation with non‐hazardous solid waste. Return copies of the completed manifest to the Contracting Officer, PAR, and Environmental Hazardous Waste Manager for accountability and reporting purposes.

c. Training. Personnel handling hazardous waste must have all requisite training required by RCRA, OSHA, and DOT regulations. Contractor personnel training records must be available for review by the PAR.

d. Reporting/Documentation. Prior to the commencement of work, the contractor must submit in writing copies of Material Safety Data Sheets (MSDS), index of all anticipated hazardous materials used throughout contract performance, methods to protect contractor personnel and property during transit of hazardous and non‐hazardous material, spill response procedures, disposal procedures, and a waste management training plan. The contractor must receive Contracting Officer approval on all reporting or documentation at least one week prior to onsite commencement of work.

10. DISPOSITION OF REMOVED MATERIALS

Except where specified otherwise herein, all salvable materials disconnected or removed during work performance which are not specified in the contract for reuse shall become the property of the contractor and shall be removed from Government property. All trash, debris, and unusable materials shall be cleaned up at the end of each workday and disposed of by removing completely from Government property.

11. INSTALLATION REGULATIONS

Contractor personnel employed on NAS Meridian, MS shall become familiar with and obey installation regulations, shall keep within the limits of the work (and avenues of ingress and egress), and shall not enter any restricted areas unless required to do so and cleared by the proper authority for entry. The contractor shall conspicuously mark its equipment for ease of identification.

12. INSPECTION AND ACCEPTANCE OF WORK

Services rendered under this contract are subject to Government inspection during the performance of services and after the completion of tasks. All unsatisfactory or incomplete work shall be made satisfactory to the Contracting Officer or an appointed representative to consider the final work product to qualify as Government accepted work. All costs associated with rework are the responsibility of the contractor. The contractor shall accompany the Contracting Officers Representative (COR) on a joint final inspection of all completed work prior to leaving the base. This joint inspection will be held during normal hours, 0700‐1530, Monday through Friday. It is the contractor’s responsibility to schedule all inspections with the Contracting Officer’s appointed inspector. No payment will be authorized until a joint inspection is made and any noted deficiencies corrected. See FAR 52.246‐4 for further details regarding the inspection of services.

13. INSTALLATION ACCESS

Any contractor, vendor or supplier seeking access to NAS Meridian after 30 June 2017 are required to obtain a Defense Biometric Identification System (DBIDS) credential. DBIDS credentials are only valid for a period of 6 months, after which time the DBIDS credential expires. To avoid interruptions in accessing the installation it is important that DBIDS credentials are renewed several days prior to the date of expiration. The installation Pass and Tag Office processes DBIDS requests on behalf of the installation. The hours of operation for NAS Meridian’s Pass and Tag Office are Monday through Friday, 6:00am to 3:30pm, with the exclusion of federally observed holidays when the office is closed. The Contracting Officer and PAR should be contacted to assist in coordinating base access requests with the Pass and Tag Office.

To obtain a DBIDS credential the following items are required:

1. Present a letter or official document from my Government sponsoring organization that provides the purpose for your access.

2. Present valid identification, such as a passport or Real ID Act‐compliant state driver's license.

3. Present a completed copy of the SECNAV 5512/1 form to obtain your background check.

4. Upon completion of the background check, the Visitor Control Center representative will complete the DBIDS enrollment process, which includes your photo, fingerprints, base restrictions, and other assessments; after all preceding items are complete, an individual will receive a new DBIDS credential.

14. STATE AND LOCAL TAXES

The contractor must include all state and local taxes in its proposed contract price. No adjustment to the final contract price is permitted after contract award to compensate for the omission of taxes from a contractor’s bid price. All work performed in conjunction with this contract occurs within the state of Mississippi. It is important to refer to the Department of Revenue for the State of Mississippi to understand tax consequences and obligations for conducting business within the state. The Mississippi Department of Revenue website is https://www.dor.ms.gov/.

15. PRE‐PERFORMANCE CONFERENCE

Prior to commencing work, the contractor shall coordinate a pre‐performance conference with the Contracting Officer and PAR, to discuss and develop a mutual understanding relative to scheduling and administration of work.

During the pre‐performance conference the Contracting Officer and PAR will review installation and federal administrative regulations, along with WAWF invoice procedures.

16. SITE VISITATION

The additional service requirements incorporated through Amendment 0001 impact the performance work statement to a degree that warrants scheduling a second pre‐proposal site visit. The second pre‐proposal site visit will allow potential offerors the opportunity to personally view the underground collection tank at the dinning facility, as well as, the future sites of the grease collection units needed at fire stations 1 & 2. The pre‐proposal site visit will be held at the NAS Meridian Contracting Office in building 427 on Wednesday, April 10th at 0900. If you plan to attend the in‐person site visit please notify the Contracting Officer, Lea Cook, by e‐mail at lea.m.cook.civ@us.navy.mil or by telephone at 601‐679‐2804. Please consult sections 13. and 16. for additional guidance on how to obtain access to installation.

Prior to submitting a contract offer, the Government will invite all prospective bidders to visit the dinning facility, inspect the grease collection tank, and satisfy themselves as to the degree and complexity of the maintenance services required by the Government to execute the contract scope of work. The Government encourages all prospective bidders to attend the scheduled site visit to satisfy themselves as to the systems, locations and requirements involved to accomplish the work specified in the solicitation. Access to the station will be dependent on a federal background check to authenticate that an individual obtained adequate security clearance. The Government recommends that a contractor pre‐screen its employees whom they wish to attend the site visit to mitigate any situation that might arise if an employee fails to obtain adequate security clearance. The Government is not responsible for delays encountered by the contractor due to the security verification process.
