About this file
This document is a Past Performance Information Request and Questionnaire issued by the Naval Information Warfare Center – Pacific for use in evaluating contractors on a competitive service contract. The contracting officer, Jacob Ward, requests that references named by an offeror complete the attached survey form and submit it by the proposal due date to david.m.roden2.civ@us.navy.mil. The questionnaire is to be completed by government or customer technical representatives who have direct knowledge of a contractor's performance under a prior contract.
The survey evaluates contractor performance across six key areas: (1) Quality – conformance to specifications, accuracy of reports, and technical excellence; (2) Schedule – timeliness of performance relative to milestones and delivery schedules; (3) Cost Control – effectiveness in forecasting and managing contract costs, billing accuracy, and cost efficiency; (4) Management – integration and coordination of contract activities, problem identification, corrective action, customer satisfaction, responsiveness, and personnel management; (5) Utilization of Small Business – compliance with FAR 52.219-8 and FAR 52.219-9 regarding small business participation; and (6) Regulatory Compliance – adherence to all applicable regulations, codes, and reporting requirements. Respondents rate performance using five categories: Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory. Per FAR 15.305(a)(2)(iv), offerors without relevant past performance records receive a neutral rating rather than favorable or unfavorable evaluation.
