SUMMARY OF CHANGES
SECTION A - SOLICITATION/CONTRACT FORM
The required response date/time has changed from 06-Sep-2019 04:00 PM to 09-Sep-2019 04:00 PM.
SECTION B - SUPPLIES OR SERVICES AND PRICES
The following have been added by full text:
QUESTIONS AND ANSWERS
1. We would like to request a one week (7 days) extension on the due date due to the effects of Hurricane Dorian on manufacturing facilities.
Answer: Offeror submission of the written proposal shall be by 9 September 2019. Section L, paragraph 1.4 is amended to provide Offeror submission of the PDMs by 13 September 2019.
2. Amendment 3 answer 6 states that the female buckle is not adjustable. It is not clear how to attach the female buckle or how far it should be from the end of the cummerbund. Can you advise how to attach the female buckle?
Answer: Please see the following:
3. The ID patch requires shade 17925 white lettering. Our supplier does not this shade readily available. For the PDM submission would it be acceptable to use shade Pantone 649U white lettering.
Answer: The Pantone 649U white lettering is acceptable for the PDM.
4. The drawing depicts the identification/instruction label and floatation warning label in a vertical line on the carrier. However, using the dimensions provided for the identification/instruction label in paragraph 3.6.1 there doesn't seem to be room for both labels. What size should each label be and which one is on top?
Answer: Drawings are for general reference. There is no requirement for the labels to be in a vertical line or in any particular order/arrangement; dimensions are per paragraph 3.6.1.
5. Paragraph 3.6.1 says that the identification/instruction label should include chest circumference.
However, the identification/instruction label example in appendix E does not depict this. Should we add the chest circumference to the label for the PDM's?
Answer: Yes, the chest circumference shall be included (see Appendix A, Table VIII-A for circumferences).
6. Appendix A, Table III-A, identifies the maximum MAS system weights. Should the ballistics and or buoyancy compensator panels be included in the MAS system weight?
Answer: The MAS system includes the ballistic and buoyancy panels ; so yes, they shall be included in the system weight.
7. We have discovered that the patterns provided for the foam buoyancy inserts do not provide the required square inches of foam shown in Table V-A. Should we build the PDMs as shown in the supplied patterns or await new patterns from NAVSEA that show the correct square inches of foam?
Answer: Make the PDM per the pattern.
8. With regards to solicitation N6133119R0017, Statement of Work, Section 3.2.1.3. Can you please clarify what constitutes "Conditional Approval" with regards to First Article Testing?
Answer: Conditional approval applies to first articles that pass ballistic and non-ballistic testing, but for which minor/easily correctable discrepancies are noted during inspection. Conditional approval requires the noted discrepancies be corrected for production articles (i.e., final approval of the item/design would be contingent upon completion of the first LAT).

0.01 inch when measured under 0.5 psi when tested in accordance with ASTM D1777.
(3) Offeror shall list material fiber types used in proposed ballistic package and where used (which layer).
FACTOR 3: MANUFACTURING CAPABILITY
The Offeror shall provide their manufacturing capability as follows:
a. The Offeror shall describe their ability to produce a minimum of 1,000 MAS units per month, commencing 60 days after FAT approval/production contract award.
b. The Offeror shall describe monthly maximum manufacturing capacity to produce the MAS.
c. The Offeror shall describe its percentage capacity required to meet SOW requirements.
d. The Offeror shall describe in detail the proposed quality assurance plan and corrective action methods and how these will be implemented during the production process to deliver a reliable product under continuous production.
FACTOR 4: PAST PERFORMANCE
The Offeror shall provide their past performance as follows:
a. Provide up to three past performance references that reflect recent, relevant experience within the last five years that demonstrate the Offeror’s (Prime) ability to handle the scope and breadth of the SOW. Three of the Prime's relevant past performances are desired, however, the Government will accept two Prime past performance references and one past performance reference from a teamed subcontractor. Example contracts may have been performed for Federal, State, and/or local Governments, as well as private industry. In addition to the Past Performance information required by Factor 4, Offerors may utilize Attachment J-4. If the offeror elects to include Attachment J-4, the information shall be completed by the past performance reference and submitted directly to the Contract Specialist.
b. Include the following information for each past performance reference:
i. Contract/Task Order number
ii. Contract type
iii. Program name
iv. Total contract cost
v. Short description of work performed
vi. Description of how the work performed is relevant to the requirements in the SOW
vii. Names and valid phone number, e-mail address, and mailing address for the Procuring Contracting Officer (PCO),
viii. Contracting Officer’s Representative (COR) and Program Manager (PM).
c. Offerors are responsible for providing contract descriptions and applicable experience in sufficient detail to permit evaluation of the example contract’s relevancy. If the Offeror does not provide up to three past performance references during the last five years, the Offeror shall address this and provide a written narrative detailing the reasons for not providing the required references.
d. The relevancy of Offerors’ past work and how this experience will assist in performing under this contract shall be explained in detail. Offerors are advised that: (1) the Government may contact any or all references in the proposal and third parties (such as other customers, the DoD Past Performance Information Retrieval System (PPIRS), other Government contractors, the Environmental Protection Agency, the Department of Labor, consumer protection organizations, etc.) for performance information; (2) the Government reserves the right to use any such information received as part of its evaluation of the Offeror’s past performance;
(3) the Government reserves the right to directly solicit input on the Offeror’s past performance from any other sources and the Government may use any other data available to its evaluators, including the personal knowledge of those individuals; and (4) if the Offeror omits projects of which the Government evaluation team is aware or becomes aware, customer assessments may be sought from the relevant organization.
e. The Government reserves the right to limit or expand the number of references it contacts and to contact references other than those provided by the Offeror. If subcontracting is proposed for subject Contract/Task Order, past performance references where the Offeror has served as a prime Contractor with Subcontractors would be of interest, if relevant.
f. If past performance is supplied for a firm whose CAGE CODE and DUNS Number does not match the CAGE CODE and DUNS NUMBER for the offeror, the offeror MUST provide a DETAILED written explanation of how the past performance information is relevant. For example, if the offeror provides past performance data from a different firm with a different CAGE CODE and DUNS NUMBER, the offeror’s proposal MUST clearly demonstrate how it will employ into this IDIQ Contract the same policies, procedures, processes, management, and similar systems and delineate how the offeror will achieve the same quality of past performance in performance of this IDIQ Contract. The proposal must clearly show how experience from any other entity other than the offeror is relevant to the offeror’s ability to perform.
The relevant considerations are whether the resources of a different firm will be provided or relied upon for contract performance such that the other firms(s) will have meaningful involvement in contract performance. Failure to provide an adequate connection and link between the offeror and the submitted past performance from a different firm(s) may result in a lower relevancy rating.
3.3 VOLUME III
FACTOR 5: PRICE PROPOSAL
a. Pricing information shall be submitted by completing the SF33 in its entirety. Offerors shall provide a price for each CLIN. $0.00 is not acceptable.
b. If any amendments to the solicitation are issued the Offeror must ensure that such SF 30 amendment(s) are signed, dated, and returned with the proposal by the submission date and time.
c. Ensure all the information for your company is current within the System for Award Management (SAM).
d. Complete and submit all required Representations and Certifications.
e. Submit a copy of the completed SF33 and signed amendments (if any).
Cognizant DCMA and ACO Offices:
