This will be conducted through a Lowest Price Technically Acceptable (LPTA) source selection process as described in FAR 15.101-2. The first task order for this requirement will be tied to the initial contract awarded to whichever awardee is deemed to have the LPTA.

a) Offer must make an affirmative statement that the contractor has successfully performed same or like requirements equivalent or similar to that listed in Attachment 01, the Seed Task Order Performance Work Statement (PWS). Specifically, offerors should include a statement expressing their capability to meet the special requirement of providing an air compressor, which is referenced in paragraph 4.0 of the PWS.

If the prospective offerors QMS has not been approved by Northwest Regional Maintenance Center the prospective offeror shall submit their QMS for review in accordance with the requirements defined in NAVSEA Standard Item 009-04.
d) That such has been verified by at least two but no more than three recent and relevant contracts.
“Recent” and “Relevant” is defined in Factor Two. It is anticipated that these recent and relevant contracts provided in Factor One, Technical Capability, will be the same contracts provided in Factor Two, Past Performance;
2. FACTOR TWO: PAST PERFORMANCE
Offerors shall complete Previous Contracting Efforts Narrative Form (Attachment 02) for at least two but not more than three (3) of their most recently completed contracts (not to exceed three years since completion) for supplies/services that are similar in scope, magnitude and complexity to this procurement.
Offerors are encouraged to submit brief and concise responses but may expand the form to provide sufficient explanation. Data submitted other than that requested on the Contractor Performance Data Sheets will not be considered. Failure to submit Contractor Performance Data Sheets shall be considered certification that the contractor has no past performance for the Government to evaluate.
The offeror shall provide information for at least two (2) but no more than three (3) previous contracts (to include task orders or delivery orders) completed with a Contractor Performance Assessment Reporting System (CPARS) record (if accomplished) where work was performed by the offeror as either the prime or a significant subcontractor whose effort was relevant. To be considered a significant subcontractor, the offeror’s submitted previous contract documentation must demonstrate evidence of exceeding $12,000 or more per year, or 10% of the total contract price. To be considered recent, the contract(s) provided should be performed within the three (3) years from issuance date of this solicitation.
To be considered relevant, the offeror’s submitted contracts shall provide documented experience demonstrating the ability to accomplish the requirement presented in the PWS and this request for proposal
(RFP).
Offerors may submit contracts currently being performed as long as a minimum of two (2) months of performance has been completed as of the closing date of this RFP. Summary lists of contracts or incomplete data will not be considered.
If your firm has no past performance, affirmatively state this fact in your proposal.
The Government may verify past performance information from any source, to include past performance for contracts not submitted by the offeror. The Government may contact some or all of the references provided, as appropriate, and may collect information through questionnaires, telephone interviews, information on hand, and existing data sources to include but not limited to CPARS.
Note:
It is incumbent upon Offerors to ensure they are providing valid names, phone numbers and e-mail addresses. References that cannot be contacted may result in a neutral rating;
Offerors should list customer references who are most familiar with their performance on each of these contracts. The references should be persons involved in performance under the contracts such as: project managers, contracting officer’s representatives, and marine surveyors. (Do not list the procurement contracting officer).
Offerors shall provide a narrative explanation on each of the proposed past performance efforts describing how they are similar in scope, magnitude or complexity to the type of work being solicited, or that your firm has no relevant experience.
3. FACTOR THREE: PRICE
The offeror shall provide a total price included in their cover letter to be evaluated by the Government.
The offeror shall provide the completed Price Cost Breakdown sheet found in Attachment 4. Round your costs to whole numbers, i.e. $100.50 = $101 or $100.49 = $100.
