Draft Agenda for the Site-Visit (25 -28 April 2023)

Location: Main Base (Meet at Nimitz Park Parking Lot)
(2) Date and Time: 26 April 2023, 09:00 AM Location: POL Area (Meet at the Akasaki POL Depot Main Gate)
(3) Date and Time: 27 April 2023, 09:00 AM Location: Ordnance Area (Meet at Nimitz Park Parking Lot)
(4) Date and Time: 28 April 2023, 09:00 AM Location: Hario Housing Area (Meet at the Hario Housing Main Gate)
NOTE:
1. The Government will not provide transportation for the site visit.
2. Individuals are required to have a “DBIDS Card” (Base Access Pass) for CFAS Main Base, Main Base Piers, Hario Housing, Akasaki POL, Iorizaki POL, Yokose POL, Maebata Ordnance Area, and Harioshima Ordnance Area for the event.
3. All attendees required to submit following information to the CS (Keiko Ujiie, Keiko.ujiie2.ln@us.navy.mi) via email no later than 10:00 AM on 18 April 2023.
(1) Attendee’s name and Company’s name
(2) Copy of DBIDS Card for all area
(3) Copy of Vehicle Pass to access all area
(4) Copy of Shaken-sho (車検証) and Jibaiseki (⾃賠責保険)
4. If you do not have a DBIDS Card for CFAS Main Base, Main Base Piers, Hario Housing, Akasaki POL, Iorizaki POL, Yokose POL, Maebata Ordnance Area, and Harioshima Ordnance Area, please submit an access request with the following documents to the CS (Keiko Ujiie, Keiko.ujiie2.ln@us.navy.mi) via email no later than 10:00 AM on 05 April 2023 in order to receive a DBIDS Paper Pass (Temporary Pass).
For Japanese National:
(1) DBIDS Card-Paper Pass Request (Attachment (a))
(2) SECNAV5512-1 (Attachment (b))
(3) Privacy Act Statement (USFJ196aEJ) (Attachment (c))
(4) Privacy Act Statement (USFJ196bEJ) (Attachment (d))
(5) Certificate of Residence (住⺠票 本籍、家族記⼊あり)
(6) Copy of picture ID (Valid Japanese Passport, Japanese My-Number-Card or Japanese driver's license
For Non‐Japanese National:
(1) DBIDS Card-Paper Pass Request (Attachment (a))
(2) SECNAV5512-1 (Attachment (b))
(3) (Japanese resident) ‐Valid Passport and either a Japanese Resident Card or Japanese driver’s license.
(4) (Not a Japanese resident) ‐Valid Passport.
5. Pass applicants must carry picture identification (ID). Valid picture ID for Japanese Personnel means a Driver’s License, a Passport or a Resident Card with picture (Jyuki Card). A valid picture ID for non- Japanese Personnel means a Passport or Alien Registration Card.
6. Vehicle Pass applicants must submit a copy of the Japanese Vehicle Registration (Shaken-sho), a copy of the Compulsory Automobile Liability Insurance (JCI - Jibaiseki Hoken Sho), and a color copy of the driver’s license to obtain the Temporary Vehicle Pass (TVP).
7. CS will inform you the date and time for pick-up a Temporary Pass. In order to receive the DBIDS PAPER PASS (Temporary Pass) and/or the Temporary Vehicle Pass (TVP), the applicant must be escorted from the CFAS Back Gate by Government Personnel to the Pass Office. There is no parking at the CFAS Back Gate. Utilize Public Transportation, park at a public parking location near CFAS and walk to the CFAS Back Gate, or be dropped off at the CFAS Back Gate where the Government Personnel will meet the applicant.
8. Participants are required to wear Personnel Protection Equipment (PPE) such as Hard Hat, Safety Shoes and Reflective Vest. It is the sole responsibility of each attendee to wear any Personal Protection Equipment (PPE) in accordance with the latest EM 385-1-1 (Safety and Health Requirements Manual) they determine appropriate for the site visit. The U.S.
Government/Navy will not provide any PPE for persons attending the site visit. If proper PPE is not used, the individual may not be allowed to participate in the site visit to the full extent.
