About this file

This document is a Past Performance Questionnaire to be completed by a customer evaluator for a federal contractor. The questionnaire covers the contractor's performance in areas such as providing experienced managers, following quality control procedures, meeting schedules, and responding to contract changes. The customer evaluator is asked to provide an adjectival rating (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) for each performance area and include narrative explanations for any Marginal or Unsatisfactory ratings. The questionnaire also includes questions about whether the contractor was issued a cure or show cause notice, if the customer would award another contract to the contractor, and if the contract is rated in CPARS/PPIRS. This questionnaire is intended to be submitted along with the contractor's proposal in response to a federal contract opportunity.

The related federal contract opportunity is for an Indefinite Delivery, Indefinite Quantity (IDIQ) Firm-Fixed Price contract to perform repair and replacement of interior decking on naval vessels at Naval Station Mayport, Florida. The required work includes repairing or replacing carpet, ceramic tile, electric grade matting, broadcast quartz, wear resistant decking, and cosmetic polymeric decking, as well as various types of deck paint. Interested parties must have a minimum of 2 years of marine decking work experience.
