About this file
This is a Quality Assurance Surveillance Plan (QASP) for the NCTAMS LANT Pensacola Division contract covering telecommunications Operations and Maintenance (O&M) support services.
The contract requires the contractor to provide maintenance support to telecommunications voice and data circuits beyond established demarcation sites utilizing existing copper and fiber infrastructure. The QASP establishes performance standards and surveillance methods across multiple service categories: OSP Response Time (routine, major, and catastrophic incidents requiring 96-98% resolution rates), Scheduled/Preventive Maintenance (98% resolution rate for BellSouth-owned cable infrastructure performed during normal business hours per OEM guidelines), Contract Deliverables (100% inspection with >90% timely submission without rework), Overall Contract Performance (satisfactory annual rating), Invoicing (monthly with >98% accuracy), and Combating Trafficking in Persons (CTIP) compliance (100% accuracy). Response times range from next business day by 1200 hours for routine calls to 2 hours for catastrophic incidents affecting 95% or more of service. The Government monitors performance through contractor metrics, spot checks, inspection by COR, and customer surveys. Non-compliance results in Contract Discrepancy Reports (CDRs) requiring written contractor response within 5 days with corrective action plans. Failure to achieve satisfactory annual performance ratings, as determined by the Contracting Officer Representative's CPARS report, may result in contract termination, loss of future Government contracts, and non-exercise of available options.
