Firm Fixed Price (FFP) Level of Effort (LOE) Task Order 1 - CLIN 0001

Statement of Work (SOW) for crane and rigging services in support of Missile Service Unit

(MSU) System Verification Testing at Strategic Weapons System (SWS) Ashore, CX 25, CCAFS

1. Requirement

1.1 Strategic Systems Programs (SSP) requires crane lift support services, including engineering, crane, rigging, and staging support, to accomplish a complex vertical lift crane operation at Cape Canaveral Air Force Station (CCAFS), Complex 25. The first lift will require the lifting of the Missile Service Unit (MSU) off of the MSU trailer or the MSU

Storage Pad and placing onto the top of the BLDG 51902. The follow on lifts will require lifting the MSU and rotating 90 or 180 degrees and placing back down on top of the BLDG

51902 (as specified in section 8). There will also be several ground level lifts with weights less than 10K lbs required.

1.2 The primary lift requires an approximately 332” long by 272.5” square object weighing approximately 54,500 lbs. to be raised via 4 point lifting sling, lifted from ground level to the top of a facility at a height of approximately 30 feet.

1.3 During conduct of these handling operations, crane operators and riggers will be working in conjunction with Navy civilian, active duty Navy, and other Navy Contractor personnel.

2. Period of Performance

2.1 The contractor shall provide crane certification documentation to the SWS Ashore

Weapons Officer and Lockheed Martin 30 calendar days prior to the evolution (6/23/2018).

The Crane mobilization period for the lifts in steps 8.1-8.4 is 7/23/2018-10/8/2018.

Assume the crane will be actively manned for operations for 25% of this timeframe. MSU removal from the SWSA Superstructure takes place in step 8.5, likely during a second mobilization period 8/6/2019.

3. Security

3.1 All Security requirements from the Indefinite Delivery Indefinite Quantity (IDIQ) contract

N00030-18-D-0001 shall apply.

4. Safety

4.1 All Safety requirements from the IDIQ contract N00030-18-D-0001 shall apply.

4.2 In addition to other applicable requirements, the following shall apply to this effort:

4.2.1 Shift supervisor will attend the 1500 Daily Productions Meetings.

5. Crane Services

5.1 All crane service requirements from the IDIQ contract N00030-18-D-0001 shall apply.

5.2 The contractor shall coordinate a time with the SWS Ashore Weapons Officer, Mr. Scott

Brockman or the SWS Ashore Assistant Weapons Officer, Mike Ansley to perform a preliminary site inspection and develop a lift plan. The Contractor shall submit a Complex

Lift Plan to Naval Ordnance Test Unit (NOTU), Facilities Division (SPP12) .

5.3 The weight of all Government Furnished Equipment (GFE) is provided in Paragraph 7.

5.4 During the performance of the effort described in this SOW, the contractor’s crane supervisor shall communicate directly with Mr. Scott Brockman or Mike Ansley.

6. Rigging Services

6.1 All crane service requirements from the IDIQ contract N00030-18-D-0001 shall apply.

8. Lifts

8.1 Lift the MSU from the MSU Trailer or MSU Storage Pad and position the NW corner to provide access to the SWSA TB1.

8.2 Lift the MSU from SWSA TB1 and re-position it such that the NE corner is positioned over SWSA TB1.

8.3 Lift the MSU from SWSA TB1 and re-position it such that the SW corner is positioned over SWSA TB1.

8.4 Lift the MSU from SWSA TB1 and re-position it such that the SE corner is positioned over

SWSA TB1.

8.5 If necessary, lift the MSU from SWSA TB1 and re-position it back on the MSU Trailer or the MSU Storage Pad.

Representative Task Order FFP LOE 2 - CLIN 0002

Statement of Work (SOW) for Crane, Rigging in Support of MSU removal during or before

HURRICANE CONDITION 3 at SWS Ashore, CX 25, CCAFS

1. Requirement:

1.1 SSP requires crane lift support services, including engineering, crane, rigging, and staging support, to accomplish a complex vertical lift crane operation at CCAFS, Complex 25. The lift will be in the event of a hurricane to remove the Missile Service Unit (MSU) off the facility and place on the MSU storage pad or MSU transport trailer. Upon or prior to

Hurricane Condition (HURCON) 3 (Severe winds probable in 72 hours) contract company could perform lift with 24 hours of declaration. After the hurricane threat clears replace the

MSU to the top of BLDG 51902 if directed by the SWS Ashore Weapons Officer or SWS

Ashore Assistant Weapons Officer.

1.2 The primary lift requires an approximately 332” long by 272.5” square object weighing approximately 54,500 lbs. to be raised via 4 point lifting sling, lifted from the top of a facility at a height of approximately 30 ft. to a storage pad or MSU transport trailer at ground level.

1.3 The crane, rigging, and staging services will be required during any time the Cape

Canaveral Air Force Station (CCAFS) is at HURCON 3 and the MSU is on top of the SWS

Ashore Test Facility located at Complex 25.

2. Period of Performance (POP)

2.1 1 Day

3. Security

3.1 All Security requirements from the Indefinite Delivery Indefinite Quantity (IDIQ) contract

4. Safety

4.1 All Safety requirements from the IDIQ contract N00030-18-D-0001 shall apply.

5. Crane Services

5.2 The contractor shall coordinate a time with the Mr. Scott Brockman or Mike Ansley to perform a preliminary site inspection and develop a lift plan. The Contractor shall submit a

Complex Lift Plan to NOTU Facilities Division (SPP12).

5.2 The weight of all Government Furnished Equipment (GFE) is provided in Paragraph 7.

5.3 During the performance of the effort described in this SOW, the contractor’s crane supervisor shall communicate directly with Mr. Scott Brockman or Mr. Mike Ansley.

6.Rigging Services

6.1 All crane service requirements from the IDIQ contract N00030-18-D-0001 shall apply.

Representative Task Order 3 FFP LOE - CLIN 0003 (Studies)

Statement of Work (SOW) for Engineering Support Services for Crane, Rigging, and Staging

Requirements at CX 25, CCAFS

1. Requirements

1.1 Six (6) months prior to evolutions at SWS Ashore that require coordination (e.g. GFE

Delivery, testing, AIM, MTS, and MSU/SU handling operations), the manager in charge of the operation will develop an agenda and schedule a kick-off meeting that will be attend by the project leads and cognizant managers. The crane provider shall attend this meeting as an active participant and assist with the scheduling and lead times and deliveries of equipment under their purview. Site visits and analysis may be required to understand location of the crane pads as compared to reach of the crane and corresponding item to be lifted. The crane contractor will attend one, four hour long meeting, six months prior to the handling event.

1.2 Two (2) months prior to evolutions at SWS Ashore that require coordination (e.g. GFE

Delivery, testing, AIM and MSU/SU handling operations), project leads and cognizant managers will attend weekly SWS Ashore coordination meetings. The crane provider shall attend these meetings as an active participant and assist with the scheduling and lead times and deliveries of equipment under their purview. Site visits and analysis may be required to understand location of the crane pads as compared to reach of the crane and corresponding item to be lifted. The crane contractor will attend one, four hour long meeting two months prior to the handling event.

1.3 The contractor shall support up to four dry runs or slow run-throughs of complex lifts at

SWS Ashore. These evolutions are simulated and involve table-top proofing of procedures and safety instructions. No crane is required but the contractor’s involvement is required to interface with the Navy’s team to ensure safe systems of work are in place in support of lifting/handling military equipment. The purpose of the slow run-throughs is to table-top the Navy’s lifting and handling procedures for familiarization before the evolution takes place. The crane company’s project manager and crane operator will interface with the

SWS Ashore operations crew and handling teams from the Navy shipyards to establish a safe system of work prior to the primary lift. The crane contractor’s team will be on site for two days during a one week timespan, Monday-Friday, 8 hours, per person, per day, to support the proofing of Navy procedures.

2.1 Estimated POP of 180 days.

Representative Task Order FFP LOE - CLIN 0004 and CLIN 0005

Statement of Work (SOW) for Crane and Rigging Services for AIM Handling SWS Equipment and Heavy Haul Transportation

1. Requirement:

1.1 Strategic Systems Programs (SSP) requires crane lift support services, including engineering, crane, rigging, and staging support, to accomplish a complex vertical lift crane operation at Cape Canaveral Air Force Station (CCAFS), Complex 25.

1.2 The primary lift requires an approximately 68ft long by 97 in. cylindrical object weighing approximately 170K lbs (the “cylinder”) to be raised from a horizontal to a vertical position via engineered trunions at the pivot point, at ground level, rotated 180⁰, then lifted to the top of a facility at a height of approximately 30ft. The cylinder would then be rested temporarily atop the facility with a slacked hook, then returned to ground level and restored to a horizontal position. The cylinder would be approximately 90K when being returned to the horizontal position.

1.3 The resting point atop the facility for the primary lift is approximately 45ft from the west end of the facility. Approach to the facility to perform the lift must come from the west end.

1.4 Prior to the primary lift there will be several ground level lifts ranging from 2k to 42K lbs.

These lifts will be in support of configuring the cylinder with a hoisting motor to raise and lower a component internal to the cylinder and installation/removal of shipping, stowage, and loading equipment.

1.5 There will be numerous lifts to the top of the facility ranging from 5K to 20K lbs to install/remove support equipment. These lifts will occur prior to and after the primary lift.

Once the cylinder is returned to the horizontal position on ground level, additional lifts will be required, in the reverse order of para 1.4 above, to de-configure the cylinder.

1.6 The sequence/process of events will be in accordance with approved local handling procedure.

1.7 During conduct of TUR-46-06 (AIM Handling Procedure), crane operators and riggers will be working in conjunction with Navy civilian, active duty Navy, and other Navy Contractor personnel.

1.8 Crane, rigging, and staging services will be required for 8 days prior to the primary lift and

4 days following the primary lift. This time frame does not include mobilization or demobilization. The Engineering Services portion will span the complete period of performance.

1.9 During the Crane mobilization period, the contractor shall provide a JLG-type man-lift in support of hoist installation and removal of the AIM or MTS.

2.1 Estimated POP of 75 days.

4. Safety

4.1 All Safety requirements from the IDIQ contract N00030-18-D-0001 shall apply.

5. Engineering Support

5.1 All engineering support requirements from the IDIQ contract N00030-18-D-0001 shall apply.

5.2 The Contractor shall provide engineering services in support of crane, rigging and staging requirements, as required by the TUR-46-06 (AIM HANDLING PROCEDURE). These services shall include the development of the following:

5.2.1 Lay down area

5.2.2 Stay out zones

5.2.3 Lift plans

5.2.4 Rotation arcs

5.2.5 Engineered site/lift drawings

5.2.6 Staging design

5.3 The contractor shall coordinate a time with Mr. Scott Brockman or Mr. Mike Ansley to perform a preliminary site inspection and develop a lift plan.

5.4 The Contractor shall participate in up to four two-hour on-site planning meetings to provide crane and rigging expertise during the period of performance. The Government will provide, at a minimum, 5- calendar day advance notice to the Contractor of such events and meetings. Attendance of some meetings may be by TELCON, at Governments discretion.

6. Crane Services

6.1 All crane services requirements from the IDIQ contract N00030-18-D-0001 shall apply.

6.2 The Contractor shall perform the primary lift and other lifts, as described in paragraph 1, above, subject to the following:

6.2.1 The cylinder cannot be traveled once the load is in a vertical position.

6.2.2 The cylinder may be rotated and boomed.

6.2.3 If unable to adhere to the no travel restriction, the Contractor shall notify the Government of the details of direction and distance and obtain a formal waiver in advance of the primary lift.

6.3 The Contractor shall assemble the crane on-site. The Contractor shall ensure that the crane is weight-tested prior to conduct of any operational lifts. Proof of weight test will be provided to the Government. The weight of all Government Furnished Equipment (GFE) is provided in Paragraph 9, below.

6.4 The Contractor will provide 3 calibrated inline Load Indicating Devices (LIDs), with remote readouts. The range of the 3 devices must be 10K, 50K, and 200K.

6.5 During the performance of the effort described in this SOW, the Contractor’s crane supervisor shall communicate directly with the assigned Navy Test Director or Handling

Supervisor.

6.6 During the crane service portion of this effort, the Contractor’s crane supervisor(s) shall attend all daily handling evolutionary briefs.

6.7 The Contractor will provide the Government with a complete list of required services (e.g., electrical power, office space, storage, etc.) on the date of contract award.

7. Rigging Services

7.1 All rigging service requirements from the IDIQ contract N00030-18-D-0001 shall apply.

7.2 Contractor provided general rigging gear shall have evidence of satisfactory weight test.

Some of the GFE listed in Paragraph 9 of this SOW has requirements to use specifically designed slings and rigging gear. These specific slings and rigging gear will be provided to the contractor along with evidence of satisfactory weight test. The Contractor will provide the following:

7.2.1 Various wood dunnage up to the size of 12 inch x 12 inch x 4 feet (8 pieces).

7.2.2 Chain falls ½ ton, 2 ton and 5 ton (2 each)

7.2.3 3 Swivel Hoist Rings (.500-13 UNC)

7.3 During the rigging service portion of this SOW, the Contractor’s riggers and supervisor(s) will attend all morning briefs.

8. Staging Services

8.1 All staging services requirements from the IDIQ contract N00030-18-D-0001 shall apply.

8.2 The Contractor shall provide a Staging Section, which must adhere to the following specifications:

8.2.1 U shaped with two (2) lengths of approximately 18’ feet and the cross-member approximately 9’ long. The staging must have a minimum 18” work platform at approximately 6ft high and OSHA standard safety rails along the outside perimeter. The staging will be on wheels and lockable. The staging can be moved as a unit or in sections but must have safety rail continuity when in place.

8.2.2 Two sections approximately 8’ long on locking wheels with a minimum 18” work platform approximately 7’ high and OSHA standard safety rails around end and 1 length.

8.2.3 Both sets of staging must have fall restraint tie off points along the perimeter.

8.2.4 Both sets of staging must be capable of support 3 workers and common tools

8.2.5 Both sets of staging must have ladder access and swing gates.
