Cost Information: For cost plus incentive fee (CPIF) contracts, identify the targeted costs, cost at completion (or current estimate for completion), minimum fee and earned fee (or anticipated earned fee). For cost plus fixed fee (CPFF) contracts, identify the initial estimated costs, fixed fee, cost at completion (or current estimated cost at completion), and effective/earned fee. For cost plus award fee (CPAF) contracts, identify the initial contract estimated costs, award fee pool, award fee scores, earned fee, and cost at completion (or current estimate at completion). For fixed price type contracts, identify the initial award price and the final price at contract completion, and if applicable provide details with respect to each contract change in price awarded during the performance of the contract.
Provide a description of the contract services and/or products required and provided.
Self-Assessment: Provide a self-assessment of performance for this contract. The self-assessment should address (1) the quality of services and/or products provided; (2) the timeliness of performance; (3) control of costs associated with the contract; and (4) customer satisfaction. Additional information can be provided with respect to changes in scope of work, cost growth, material problems, manufacturing problems, quality problems, labor problems, facility problems, delays in the schedule, and any other issues experienced. The Offeror should demonstrate how they were able to anticipate, overcome and/or resolve any special or unexplained problems as well as difficulties in meeting delivery schedule, performance or cost requirements. The Offeror should indicate any quality awards, performance awards or recognition received.
Customer References: The Offeror shall partially complete, and request customers fully complete and submit the Past Performance Reference Questionnaire associated with this information sheet.
