Photographic Documentation Summary

This is a photographic documentation file for the City of New Smyrna Beach's Hazard Mitigation Grant Program (HMGP) project to elevate six flood-prone single-family homes in Volusia County, Florida. The documentation contains project site photographs of the six properties subject to elevation: 455 DeSoto Drive, 645 Goodwin Avenue, 647 Goodwin Avenue, 508 South Indian River Road, 529 South Peninsula Road, and 640 South Pine Street. These homes are located in Special Flood Hazard Areas (SFHA) classified as Zone "AE" and have finished floor elevations below the Base Flood Elevation (BFE) established by FEMA, placing them at imminent risk of recurring flooding. The project involves comprehensive turnkey services including structural assessments, boundary surveys, geotechnical assessments, construction plans, permitting, utility disconnection and reconnection, and elevation of structures to 10 feet above existing ground level. A non-mandatory pre-bid meeting is scheduled for December 9, 2025, at 2:00 PM via Microsoft Teams, with written questions due by December 18, 2025, at 5:00 PM through the City's eProcurement portal. Bids are due electronically by January 8, 2026, at 2:00 PM, with bid opening at 3:00 PM the same day. Work must be completed by October 31, 2026, in compliance with FEMA grant requirements and various building and flood mitigation standards.

Bid security of 10% of the total bid amount is required, with contractors obligated to maintain Workers' Compensation insurance, Commercial General Liability ($1,000,000 per occurrence), Automobile Liability ($1,000,000 combined single limit), and Professional Liability ($1,000,000) coverage. The project is 75% federally funded through FEMA's Hazard Mitigation Grant Program (Contract No. 4337-373-R-b), with the remaining 25% provided by the City of New Smyrna Beach and homeowners. Liquidated damages are set at $1,694 per calendar day for work not completed by the specified deadline. The City encourages participation from disadvantaged business enterprises, requiring contractors to take affirmative steps to include minority business enterprises (MBEs), women's business enterprises (WBEs), veteran-owned businesses (VOBs), and labor surplus area firms. Contractors must be registered with the Florida Department of State, licensed in Volusia County, maintain active Sam.Gov registration, and comply with E-Verify requirements.
