About this file

This document is a Proposal Cover Sheet for a Mini-RFP (Request for Proposal) issued by the Oregon Department of Transportation (ODOT) for Project K22706, specifically a construction project at OR6 Wilson River Loop. The project appears to be a California/Construction Engineering and Inspection (CA/CEI) procurement, with the cover sheet serving as a mandatory certification document for potential bidders. The document requires proposers to confirm compliance with various state requirements, including addenda acknowledgment, non-discrimination policies, sexual harassment prevention, debt status, and conflict of interest guidelines.

The cover sheet includes critical certifications that proposers must complete, such as verifying no outstanding state debt, accepting all Mini-RFP terms, and confirming no conflicts of interest. Notably, for contracts anticipated to be $150,000 or more, proposers must have a written policy preventing sexual harassment and discrimination. The document also requires proposers to disclose any potential conflicts of interest, including recent employment history with ODOT, and mandates that an authorized representative approve and certify the proposal's accuracy and completeness by signing the document.
