About this file
This document is a Medical Review Analysis Report template used by the U.S. Office of Personnel Management (OPM) for Federal Employee Health Benefits (FEHB) program medical reviews. The template provides a comprehensive framework for medical specialists to conduct detailed reviews of disputed health benefit claims, requiring a structured analysis that includes patient demographics, clinical findings, key events, and specific determination of service/benefit requests.
The template mandates reviewers provide peer-defensible rationales based on clinical records, current medical evidence, and clinical practice guidelines. Key requirements include documenting the medical issue under review, sequentially reporting clinical events, consecutively reporting clinical findings, and providing a specific determination that cites relevant medical literature. The template also requires a signature page with the reviewer's professional credentials, board certifications, specialty information, and a conflict of interest disclosure, ensuring a thorough and transparent review process for federal employee health benefit claims.
