About this file
This is a Mail-In Ballot Application form issued by the Maryland State Board of Elections for registered voters requesting mail-in ballots for Maryland elections. The form serves as the primary mechanism for voters to request absentee ballots for the 2026 Primary Election (June 23, 2026) and General Election (November 3, 2026), or for all future federal and state elections. Voters can select from three ballot delivery methods: U.S. Mail delivery to a residential or alternative address, email delivery of a printable ballot link, or in-person pickup at their local election office. Ballots are distributed approximately 30-45 days before each election. The form allows voters to choose their political party affiliation (Democratic, Republican, Unaffiliated, Green, Working Class, or Other) for primary election voting purposes. Submission deadlines vary by delivery method and election cycle, with mail and hand delivery deadlines of 8:00 p.m. on June 16, 2026 (Primary) and October 27, 2026 (General) for mail/fax delivery, and 5:00 p.m. on June 19, 2026 (Primary) and October 30, 2026 (General) for email/printable ballot link delivery.
The form requires voter information including name, date of birth, residential address, voter registration details, Maryland driver's license or ID card number, and last four digits of Social Security number for verification purposes. Voters can request ballot status updates via text message or email. The form permits assistance from designated individuals but prohibits aid from employers, union representatives, or candidates. Contact information for all 24 Maryland county boards of elections and the State Board of Elections is provided for voter inquiries and form submission. The related state and local contract opportunity requires printing and shipping approximately 10,500 unaddressed voting information packets to 50 correctional facilities across Maryland for both the 2026 Primary and General Elections.
