Attachment 4: Past Performance Data Sheet / Questionnaire M95494-19-R-0009

Instructions:

· Fill out 1 of these for each Past Performance reference.

· Sections I and II to be completed by the Offeror/Major Subcontractor and provided to the assessor (Government personnel who had experience with the Offeror/Major Subcontractor on a previous relevant contract). The assessor, in turn, to verify Sections I and II, complete the questionnaire (Section III), and submit to the Contracting Office. The Offeror should also submit with its proposal (by the closing date of the Solicitation) copies of Sections I and II of all questionnaires as provided to assessors.

· Section II to be validated by the assessor. If any information is incorrect herein, please annotate and provide the correct information accordingly.

· Section III to be completed in their entirety by the assessor.

Message to the assessor: Your assistance is requested by the USMC to assist with establishing the performance history for the Contractor named below. In efforts to expedite receipt of the requested information, the Contracting Office respectfully requests that you do not mail hard copies. Instead, please e-mail the completed questionnaire(s) to: Hanika Torio at hanika.torio@usmc.mil. Responses are due by:

Evaluation Definitions

The following definitions should be used in your assessment of Contractor performance.

EXCEPTIONAL
Performance meets/met contractual requirements and exceeds/exceeded many to the customer’s benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
VERY GOOD
Performance meets/met contractual requirements and exceeds/exceeded some to the Customer’s benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
SATISFACTORY
Performance meets/met contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear to be or were satisfactory.
MARGINAL
Performance does not/did not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet or did not identify corrective actions.
UNSATISFACTORY
Performance does not/did not meet most contractual requirements and if currently being performed, recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.
NOT APPLICABLE (N/A)
Performance information not recent or relevant as defined in the Solicitation. Unable to provide assessment.
