About this file

This is a Past Performance Questionnaire template for use in evaluating contractor performance on a federal contract opportunity (030ADV26R0035 – New Robot) for the Library of Congress.

The questionnaire is designated as source selection sensitive under FAR 3.104 and is intended to be completed by government personnel or references who have direct knowledge of a contractor's prior performance. Section 1 requires identification of the contract being evaluated, including the contractor name, agency contract number, contract type, competitive status, period of performance, contract cost, work description, and whether the contract experienced any terminations for default or convenience. Section 2 captures customer or agency identification and geographic scope of services. Section 3 identifies the evaluator, including name, title, phone/fax number, and years worked on the subject contract. Section 4 provides a standardized seven-point evaluation scale (ranging from N/A to 5) to rate contractor performance across seven categories: overall satisfaction, quality, schedule, cost/price management, personnel quality, contract management, and communication/problem resolution. Section 5 requests narrative responses regarding the contractor's greatest strengths and weaknesses, warranty and repair performance, support and defect remediation frequency, and whether the evaluator would hire the contractor for future critical programs. Completed questionnaires must be returned to the Contracting Specialist (Brenda Kinlay, bkinlay@loc.gov) with a copy to the Contracting Officer (James Robinson, jarobinson@loc.gov).
