Multi-Door Replacement & Repair Belcourt Service Unit

Belcourt, North Dakota C-1. Project Description

A. Title: Multi-Door Replacement & Repair

B. Location: This project is at the Quentin Burdick Memorial Health Facility, in Belcourt, ND.

C. Period of Performance: 180 calendar days from Notice to Proceed.

D. Description: The project defined within this Scope of Work is for the replacement of 15 doors and repair of 4 doors throughout the Quentin N Burdick Memorial Healthcare Facility.

E. Background: Doors throughout the Quentin N Burdick Memorial Health Care Facility are damaged, worn, and not functioning properly. This project will correct these issues through replacement and repair of a total of 19 doors.

C-2. Scope of Work. (SOW) The scope of this project includes all demo, new installation, and clean up for the replacement of the 15 doors and repair of the 4 doors indicated below and in the attached plan sheets.

The contractor shall be responsible for all necessary patching repair and painting resulting from the removal and installation of the new doors and frames. This includes, but not limited to, repairing adjacent wall surfaces, trim, and finishes as needed to match existing conditions. All disturbed surfaces shall be finished to match surrounding material in color, texture, and quality.

The contractor shall provide all engineering, design, material, labor, equipment and supervision required to accomplish the following:

A. Phasing: The contractor shall coordinate with the project engineer to phase door installation to minimize disruption to facility operations. The government reserves the right to limit the number of doors being worked on simultaneously based on operational needs, site conditions, or other considerations. The contractor shall perform the work in a manner that allows the facility to remain functional and secure throughout the duration of the project.

B. Materials: All materials shall be stored and handled according the manufacturers’ recommendations.

All materials shall be submitted to the COR prior to ordering, for approval by the project engineer.

Contractor shall not begin installation until all materials required to complete installation have arrived on site.

C. Temporary Construction Barriers: The contractor shall provide temporary partitions to separate work areas from owner-occupied areas, to prevent penetration of dust and moisture into owner occupied areas, and to prevent damage to existing materials and equipment. The contractor shall utilize a hepa-filtered air scrubber within the temporary barrier.

D. Demolition: The contractor shall remove the doors & frame as required in attached design sheets.

Demolish existing work as required to accomplish installations indicated on attached plan sheets. The removal of waste and debris is the sole responsibility of the contractor. Waste shall be disposed of in an approved landfill.

E. Installation: The contractor shall conduct an on-site verification of all existing conditions, dimensions, and other relevant project details. The contractor is responsible for ensuring all measurements, materials, and installation requirements align with the actual site conditions prior to ordering materials or beginning construction.

a. Automatic Sliding Doors (Basis of Design):

i. (TSLD) Stanley Dura-Fit: The contractor shall furnish and install four (4) Stanley Dura-Fit series Bi-Part sliding doors or approved equal at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer’s specifications and project requirements. Door finish shall be clear anodized aluminum.

All doors shall include: full breakout, 10” bottom rails, access control, and 1” insulated glazing. Doors to include, where relevant, transoms and side lites to match existing opening. Transom and side lite glazing thickness shall match door. Hardware per manufacturer’s specifications.

ii. (BSLD) Stanley Dura-Glide: The contractor shall furnish and install one (1) Stanley Dura- Glide 3000 series Bi-Part sliding doors or approved equal at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer’s specifications and project requirements. Door finish shall be clear anodized aluminum. All doors shall include: full breakout, 10” bottom rails, access control. Interior door(s) shall include 1/4” glazing. Exterior door(s) shall include 1” insulated glazing. Doors to include, where relevant, transoms and side lites to match existing opening. Transom and side lite glazing thickness shall match door. Hardware per manufacturer’s specifications.

b. Aluminum Framed Storefront Doors: The contractor shall furnish and install two (2) aluminum store front doors at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer’s specifications and project requirements. Door finish shall be clear anodized aluminum. Interior door(s) shall include ¼” glazing. Exterior door(s) shall include 1” insulated glazing. Doors to include, where relevant, transoms and side lites to match the existing opening. Transom and side lite glazing thickness shall match door. See hardware sets below.

c. Hollow Metal Doors: The contractor shall furnish and install two (2) hollow metal doors at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer’s specifications and project requirements. Door shall be factory primed and painted. Contractor shall submit from manufacturer’s range of colors for approval. Doors to include, where relevant, transoms and site lites to match existing opening. See hardware set below.

i. C027b: Door level shall be Level 3 – Extra Heavy Duty. Based on SDI standards: ANSI/SDI A250.8 (SDI-100). Door R value shall be minimum R-11.

ii. B189a: Door level shall be Level 2 – Heavy Duty. Based on SDI standards: ANSI/SDI

A250.8 (SDI-108).

d. Solid Core Wood Doors Non-Fire Rated: The contractor shall furnish and install five (5) solid core non-fire rated 20-min wood doors at the locations designated in the attached plan sheets.

The doors shall be installed in accordance with the manufacturer’s specifications and project requirements. The door finish shall be wood veneer, submit from manufacturer’s full range of finishes for approval.

e. Solid Core Wood Doors 45-Min Fire Rated: The contractor shall furnish and install one (1) 45-min fire rated wood doors at the locations indicated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer’s specifications and project requirements. The door finish shall be wood veneer, submit from manufacturer’s full range of finishes for approval.

The doors shall be installed to comply with NFPA 80 & NFPA 101.

f. Hollow Metal Frames: All non-aluminum doors shall have full profile/continuously welded hollow metal frames. Frames shall be factory primed and finished. Frame gauge shall be 16 gauge minimum. Fire rated hollow metal frames shall be labeled according to NFPA 80. See attached design door replacement schedule on sheet A.8 indicating where frames are required.

g. Door Hardware: See hardware sets below as indicated from A.8 Door Replacement Schedule on the attached plan sheets.

F. Additional Facility Requirements: Contractor shall comply with the requirements of the hospital’s Facility Manager, Infection Control Officer and Safety Officer. Contractor shall communicate with facility staff to ensure construction does not interrupt essential hospital activities. These requirements will be laid out during the pre-construction meeting. Other permits and requirements will be given after the preconstruction meeting, the infection control risk assessment (ICRA), and pre-construction risk assessment (PCRA) are completed.

A. Warranty: Provide a 1-year labor and material warranty at no additional cost to the government which provides for the repair of all work and equipment that was provided under this scope of work.

B. Clean Up: Upon completion of each door site, the construction areas involved shall be free of any debris, rubbish and remaining material scraps. Trash receptacles used for the collection of this debris and their removal from the project site shall be provided by the contractor. The site will be inspected by the engineer prior to final payment to ensure the site is returned to its original condition.

C-3. Period of Performance. The overall period of performance is 180 calendar days.

C-4. Schedule/Meetings

A. The general contractor shall submit a detailed schedule to the COR for approval within (14) calendar days of the Notice to Proceed.

B. Pre-Construction Meeting: One Pre-Construction meeting shall be conducted prior to the start of the construction of the contract.

C. Substantial Completion: One substantial completion meeting shall be held following notification of the Project Officer by the General Contractor that the project is Substantially Complete.

D. Final Completion: One final completion meeting shall be held following determination that the project is complete.

C-5. Miscellaneous

A. Other Requirements. Security: HSPD-12 clearance is required for all personnel assigned to project that will be onsite. The contractor shall be responsible for completing all necessary paperwork to obtain security clearances and badges for construction personnel who will work on the project.

a. Workers without complete security clearances will not be permitted on the work site.

b. The contractor will be responsible for the security of the work site.

B. Protection of Government Property. The contractor shall protect all Government property against damage, to include but not limited to dust, fumes, impact, scratching, water, paint, joint compound, power failure, power surges, weather, heavy equipment, etc.

a. All damage caused by the contractor, whether to equipment, roads, landscaping, building components or any other type of Government property encountered at or near the project site shall be repaired or replaced to match the existing and to the satisfaction of the Project Officer at no cost to the Government.

C. Occupancy of Government Facilities. The contractor shall be responsible for properly informing the COR of any condition which would jeopardize the building or its contents, such as loss of heat, air conditioning, danger of pipes freezing, loss of electrical power, water, sewer or loss of building security.

D. Correspondence. All correspondence shall be addressed to the COR. Each letter, document, etc., shall contain the following information:

a. Date

b. Project Numbers, Contract Number, Delivery Order Number and Service Unit Name/Location.

c. Name of those who were sent copies.

E. TERO/EPA. The contractor is required to acquire and maintain a tribal business license before any work will commence. The contractor shall comply with the Tribal Employment Rights Ordinance (TERO) and pay TERO and EPA fees.

F. HOURS OF OPERATION

The project shall start after award and receipt of the Notice to Proceed

The work hours shall be 8:00am – 5:00pm, Monday – Friday. Weekends may be available upon request.

C-6. Payments to the Contractor

a. The Government will pay the Contractor a fixed price, as indicted below, which will constitute full compensation for all Contractor professional and incidental services, travel expenses, and materials necessary to furnish complete project as required and specified in this SOW.

b. Progress payments will be paid following acceptance by the Government of that particular schedule item and a properly submitted and approved invoice.

C-7. Project Officer

Taylor Strand will be designated as the Contracting Officer’s Representative (COR) for this Order. His address and telephone are:

C-8. Invoice Format

The Contractor is responsible for delivering one copy of the invoice to the COR. The invoice shall include the contractor’s official invoice with the contractor’s letter head and a filled-out Form AAO-107C along with the contractor’s schedule of values showing a detailed breakdown of the construction items with the progress of each. The COR is responsible to approve or reject the invoice and provide a copy to the Contract Specialist and delivery of the original to the payment office.

Upon reaching 100% completion of the entire task order, submission of the final invoice must be accompanied by a Final Claims Release before payment can be approved. Failure to provide the proper information may cause the invoice to be returned or delayed.
