About this file

This document is an Asbestos Survey Report for the Colville Health Clinic in Nespelem, Washington, conducted by Federal Occupational Health in June 2008. The survey covered four buildings (00020, 00021, 00022, and 00060), with inspectors collecting bulk samples of suspect asbestos-containing materials throughout the interior and exterior. Asbestos-containing materials were identified in Buildings 00020, 00021, and 00022, including flooring, window putty, pipe insulation, duct tape, caulk, sink undercoating, plaster, and drywall joint compound. Small fragments of friable asbestos-containing pipe insulation were noted in basement areas, with recommendations for abatement or repair.

The report estimated a total cost of $167,580.00 for removing asbestos-containing materials and recommended implementing a comprehensive Operations and Maintenance (O&M) Program to manage remaining asbestos materials. The survey was performed using procedures similar to EPA AHERA regulations, with samples analyzed by a fully accredited laboratory. Most identified materials were classified as non-friable or Category I/II asbestos-containing materials, which pose minimal health risks when intact. The report emphasized that any removal or disturbance of these materials should be performed by appropriately trained and certified personnel, with disposal at approved landfills.

EXECUTIVE SUMMARY

As requested by the Portland Area Indian Health Service (illS), bulk sampling of suspect asbestos containing building materials was conducted in four (4) buildings: Building 00021 (Old Colville Health Clinic), Building 00020 (Support Building), Building 00022 (Support Building), and Building 00060 (Storage/Garage/Maintenance) located at the Colville Health Clinic area in Colville, Washington, just south of Nespelem, Washington. Sampling occurred on November 12 - November 16, 2007. Bulk samples of suspect asbestos-containing materials (ACMs) were collected throughout the interior and exterior of the buildings. Non-destructive sampling practices were utilized during the inspection.

A portion of the main level of the Old Health Clinic (Building 00021) is being used for dentistry; the basement has a maintenance storage area, an office for maintenance personnel, and a diabetes clinic. The two support bu.ildings (00020 and 00022) are being fully used as administrative offices. The maintenance shop (00060) is being used for equipment storage and ongoing maintenance work.

Asbestos-containing materials were identified throughout Buildings 00020, 00021, and 00022. A summary of samples and results of the suspect asbestos materials can be found in the tables in Appendix

2. Building 00060 was found to be asbestos free. Buildings 00020, 00021, and 00022 were found to have numerous asbestos-coutaining materials, including flooring, window putty, pipe insulation duct tape, caulk, sink undercoating, pIaster, and drywall joint compound.

Small fragments of mabie asbestos-containing pipe insulation were noted within the basement crawlspace of Buildings 00020 and 00021 and within the basement maintenance shop of Building 00021. These fragments appear to be residuals from a 1991 asbestos abatement and were located in wall cavities. The pipe insulation in the shop was open and exposed on the pipe at the ceiling level. These areas should be abated or repaired as soon as it is feasible.

Recommendations for the maintenance of the identified asbestos-containing materials can be found in Section 5 of this report.

Federal Occupational Hea\tb (FOH) representatives, Ms. Colleen McCarthy and Mr. Landon Johnson, performed the asbestos inspections of the subject properties. Each inspector holds current Asbestos Hazard Emergency Response Act (AHERA) accreditation as Asbestos Bu.ilding Inspectors. Certifications for each inspector can be found in Appendix 4. Sampling of suspect asbestos-contaiing materials was conducted using procedures similar to the Environmental Protection Agency (EPA) AHERA regulation, 40 CFR 763. Samples were analyzed by the Federal Occupational Healtb Service (FOH) Analytical LaboratoIy in Denver, Colorado. The FOH Laboratory is fully accredited by the EPA-required National Voluntary LaboratoIy Accreditation Program (NVLAP) for analysis of asbestos bulk samples using polarized light microscopy, and is also an American Industrial Hygiene Association (AIHA) accredited laboratory.

2.0 DESCRIPTION OF BUILDINGS INSPECTED

The Old Health Clinic (Building 00021) is a two-story, 17,OlO-square-fool building on a concrete slab, which was built in 1934. Building 00022 is a one-story, 3,402-square-foot building with a basement situated on a concrete slab, built in 1934. It is used as offices and the storage of files in the basement. It was constructed at the same time as the Old Health Clinic and is located east of the Old Clinic. Building 00020 is a one-story, 2,45O-square-foot building with a basement situated on a concrete slab, also built in 1934. It is used as offices and the basement is used for the storage of paints, maintenance equipment, and cleaning supplies. The maintenance garage (Building 00060) is a metal-sided 1,679-square-foot building constructed in 2002 and is located south of the Old Health Clinic. It has three bays and is situated on a concrete slab. Building maps can be found in Appendix I.

A portion of the main level of the Old Health Clinic (Building 00021) is being used for dentistry; the basement has a maintenance storage area, an office for maintenance personnel, and a diabetes clinic. The two support buildings (00020 and 00022) are being fully used as administrative offices. The maintenance shop (00060) is being used for equipment storage and ongoing maintenance work.

The Old Colville Health Clinic and the two support buildings each had a main floor and a basement. The storage building was a single-story metal-frame building.

In 1991, asbestos abatement was conducted in Building 00021. Asbestos-containing insulation from pipes was removed in the basement and crawlspace. Contaminated soil was also removed. Alliance Environmental Inc. of Federal Way, Washington, performed abatement. PBS Environmental Building Consultants ofPortIand, Oregon, provided air monitoring and on-site project management. Residnal pipe insulation is located in wall cavities throughout the building. These areas have been highlighted on a map

. and can be found in Appendix I.

3.0 SURVEY PROCEDURES

Examples of suspect building materials identified and sampled throughout the buildings included various floor tiles and associated mastics, sheet vinyl flooring and associated mastics, floor leveling compound, gypsum drywall materials., wall texture, vinyl wall coverings., stucco, brick and mortar, fiberboard ceilings, suspended ceiling tiles, various mastics, sink fireplace brick, undercoating, insulation, TSI inSUlation, covebase mastic, vibration dampener, window glazing/putty, grout, various caulks, window putty, and roof materials.

Each room of each building was assigned a unique fimctional space number. Each functional space was visually inspected for suspect asbestos-containing materials. Materials were mapped and quantified on maps provided by the rus or band-drawn maps. Each suspect ACM was designated as a distinct homogeneous area, identifYing a single material, uniform in texture and appearance, installed at one time, and unlikely to consist of more than one type or formulation of material. The inspector touched each suspect material in order to determine friability.

A sufficient number of samples were collected of each material to satisfY the Occupational Safety and Health Administration (OSHA) and the National Emission Standard for Hazardous Air Pollutants (NESHAP) regulations for the determination of asbestos content. Bulk sample logs are located in Appendix I.

Each sample was assigned a unique sample identification number and assessed for damage and friability classifications. Sampling of suspect asbestos-containing materials was conducted using procedures similar to the Environmental Protection Agency (EPA) AHERA regnlation, 40 CFR 763. The FOH Laboratmy in Denver, Colorado, analyzed the samples. The FOH Laboratory is fully accredited by the EPA required National Voluntary Laboratory Accreditation Program (NVLAP) for analysis of asbestos bulk samples using polarized light microscopy, and is also an American Industrial Hygiene Association (AlliA) accredited laboratory. Laboratory analysis reports are located in Appendix 3.

The Bulk Sample Results Table (Appendix 2) provides detailed infonnation on materials that tested positive or negative for asbestos content. A separate Bulk Sample Results Table is provided for each building inspected.

Programming Cost Estimates (Appendix 2) were developed for positive asbestos-containing materials found in each Building. The Programming Cost Estimates were derived by use of the Means Cost Guide and by consultation with experienced local contractors. The pricing reflects current industry rates. Future fluctuations in industry pricing, both natural and seasona~ may affect this estimate. In addition, Service Units outside of metropolitan areas may incur additional costs due to travel requirements on the part of abatement contractors. For purposes of this estimate, an 18% markup for logistical costs is included.

This estimate is for abatement contractor costs only. Additional costs, which are not included in this estimate, are those of an environmental consultant and replacement of building materials that have been removed. The total estimated cost for the removal of asbestos-containing materials from the Colville Health Clinic Service Unit is 5167,580.00.

5.0 RECOMMENDATIONS

In any situation where asbestos-containing material remains in a building, it should be managed under a comprehensive Operations and Maintenance (O&M) Program. The procedures and guidelines described in an O&M Program should be followed whenever building maintenance activities may disturb any ACMs present in the building.

Friable residual exposed asbestos-containing pipe insulation was noted within the basement crawlspace of Buildings 00020 and 00021 and within the basement maintenance shop of Building 0002 L This insulation was noted in small areas at the edge of wall cavities. The pipe insulation in the shop was open and exposed on the pipe at the ceiling level. These areas should be abated or repaired as soon as is feasible.

Briefly, the EPA requires that regulated asbestos-containing materia1s be removed from facilities scheduled for demolition or renovation before any activity begins that would break up, dislodge, or similarly disturb the materia1s or preclude access to the materials for subsequent removal.

7.0 PROJECT LIMITATIONS

This Project was performed using, as a minimum, practices consistent with standards acceptable within the industry at this time, and a level of diligence typically exercised by industrial hygiene and environmental consultants performing similar services.

The procedures used in this investigation attempt to establish a balance between the competing goals of limiting investigative and reporting costs and time, and reducing the uncertainty about unknown conditions. Therefore, because the findings of this report were derived from the scope, costs, time, and other limitations, the conclusions should not be construed as a guarantee that all environmental or occupational hazards have been identified and fully evaluated. Where sample collection and testing have been performed, FaIrs professional opinions are based in part on the interpretation of data from discrete sampling locations that may not represent conditions at non-sampled locations. FOH assumes no responsibility for omissions or errors resulting from inaccurate information, or data, provided by sources outside of FOH or from omissions or errors in public records. Furthermore, it is emphasized that the fmal decision on how much risk to accept always remains with the client since FOH is not in a position to fully understand all of the client's needs. Clients with a greater aversion to risk may want to take additional actions while others, with less aversion to risk, may want to take no further action.
