About this file

This is a Past Performance Questionnaire template used by the U.S. Government Publishing Office Acquisition Services to evaluate contractor performance during source selection. Offerors are required to include completed CPAR (Contractor Performance Assessment Reports) and/or PPQs as part of their quotes, not submit them directly to the Contracting Office. The information collected is classified as Source Selection Sensitive Information under 41 U.S.C. 423 and must be protected accordingly.

The questionnaire is divided into two sections: one completed by the contractor providing basic contract information (contractor name and address, project name, contract number and date, period of performance, pace of performance, completion date, contract value, and contract type), and another completed by the evaluating organization representative. The evaluator section captures the evaluator's name, position during project execution, phone number, and months of performance monitoring. The evaluation uses a standardized rating scale (EX=Exceptional, VG=Very Good, S=Satisfactory, MG=Marginal, US=Unsatisfactory, N/O=Not Observed) to assess nine performance areas: quality of products and services, overall performance, schedule, technical requirements, cost control, subcontracting, customer satisfaction in routine matters, customer satisfaction during unexpected difficulties, and repeat business likelihood. The form also requests narrative responses identifying the contractor's weak and strong points, other completed projects, and average award fee percentage earned if applicable.
