About this file
This is a Past and Present Performance Questionnaire used by the 87 Contracting Squadron to evaluate contractor performance for an Air Advisor Academics program. The questionnaire is distributed to agencies that have previously worked with prospective contractors to gather firsthand performance information during the source selection process.
The questionnaire requests responses by April 9, 2026, and must be submitted via email to Jeremy.botkin@us.af.mil and Lewis.Hampton@us.af.mil. Respondents are instructed not to send completed forms back to the offeror, as they become part of official source selection records. The form collects general information about the contractor, respondent details, and performance ratings across multiple criteria using a five-point scale: Outstanding (O), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N/A). Evaluated performance areas include management and supervision, personnel qualifications, quality control, problem resolution, responsiveness to changes, and government cooperation. Contract-specific questions address the contractor's ability to manage academic schedules and graduate students on time, proactivity in course improvement suggestions, and separate ratings for academics faculty and mission/culture training performance. The questionnaire requires narrative explanations for any Marginal or Unsatisfactory ratings and includes yes/no questions regarding cure notices, contract re-award willingness, and CPARS/CORT ratings.

Attachment 1 Past and Present Performance Questionnaire
The 87 Contracting Squadron is in the process of selecting a contractor for a program.
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies, such as yours, which have had firsthand experience with an offeror.
Our areas of interest the offeror are summarized in the enclosed questionnaire. Request that you provide your completed questionnaire no later than 9 Apr 6. You shall email your questionnaires to both and Lewis.Hampton@us.af.mil. Please email if you have any questions.
Do not send the completed questionnaire back to the offeror. Your completed questionnaire will become a part of the official Source Selection records.
Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
